REQUEST FOR AUTHENTICATION OF A LICENSE PLATE
(By the Florida Department of Highway Safety and Motor Vehicles)
This form must be completed when requesting a license plate (embossed with the years
1918 thru 1975) be authenticated for the first time by the Florida Department of Highway
Safety and Motor Vehicles for use on a car or truck of the same model year.
License plates that are submitted for authentication cannot be refurbished in any way
and must be legible for law enforcement purposes.

along with this form to expedite the process.
