EMPLOYEE BENEFITS SUMMARY 2015
Life Insurance
Carlile/Northstar Bank is committed to
our employees’ health and well-being. Our
benefits program integrates health and
wellness, providing tools and programs
to help you get and stay healthy. Carlile/
Northstar Bank and employees share in the
cost of health plan premiums. Coverage
begins the first of the month following date
of hire.
Medical Benefits
Carlile/Northstar Bank pays the full cost of Basic Life
and AD&D Insurance for employees. A Voluntary Life
and AD&D plan is available for you and your eligible
dependents, if you need more coverage. Both plans are
offered through Sun Life.
Disability Insurance
Short Term Disability and Long Term Disability plans are
provided at no cost to you. Coverage is offered through
Sun Life. Disability Insurance provides partial income
replacement if you can no longer work due to a covered
accident or illness while insured.
Additional Voluntary Insurance
Carlile/Northstar Bank offers our employees a point of
service plan (POS) through Cigna. This plan gives you
the freedom to seek care from any licensed physician or
health care professional without a referral. Preventive care is
covered at 100% in network. Most other in network benefits
are covered with a copay. In addition, we provide a Health
Reimbursement Arrangement to help with your in network
deductible. This service is administered by Higginbotham.
Accident Insurance
Dental Benefits
Critical Illness/Cancer Insurance pays a benefit after
a serious illness or a condition, such as a heart attack,
stroke, coronary artery disease or cancer is diagnosed.
Coverage under this plan is provided by Allstate.
Carlile/Northstar Bank offers you a dental plan provided by
Cigna. The plan provides excellent preventive care benefits
and basic services, as well as coverage for more extensive
treatments, including orthodontia for children.
Vision Benefits
The vision plan is provided by Davis Vision. The plan helps
pay the cost of routine eye examinations and the cost of
eyeglasses or contact lenses. The vision plan provides both
in and out of network benefits.
Accident Insurance is designed to help off-set expenses
resulting from a covered accident, such as copayments,
deductibles and other costs not covered by traditional
health plans. Coverage under this plan is provided
by Allstate.
Critical Illness/Cancer Insurance
Universal Life
Universal Life not only provides coverage in the event of your
death, it also provides financial security during life changing
events that occur as your age and your needs change. The
fund value accumulation allows for loans and withdrawals
when needed. Coverage is provided through Allstate.

Additional Benefits
Travel Assistance and ID Theft Protection
Employee Assistance Program (EAP)
Receive 24/7 emergency travel assistance for you and your
covered dependents when traveling more than 100 miles
from home, and proactive protection from identity theft
and fraud. Coverage is provided by Assist America.
Carlile/Northstar Bank provides a variety of services through
its Employee Assistance Program. Offered by Sun Life and
ComPsych, support services are offered to you and your
family at no cost. The program’s purpose is to help you deal
with personal concerns with some of life’s toughest issues.
Nurses, counselors, financial consultants and attorneys are
available through this service seven days a week 24 hours
a day. Once you open an account, you are able to receive
assistance by telephone consultations, face-to-face meetings
and reviewing education materials through an online library.
Flexible Spending Accounts
The Health Care Spending Account is a benefit Carlile/
Northstar Bank provides that lets you set aside a portion
of your paycheck into an account before paying income
taxes, thus saving you money. During the year, you can use
the funds in the account to pay for qualified health, dental,
vision, hearing and prescription drug expenses.
You can also participate in the Dependent Care Spending
Account to pay for dependent care expenses in order for
you to be able to work. Both spending account plans are
administered by Higginbotham.
Compass Professional Health Services
Compass is a service available to all employees and their families
who are enrolled in Carlile/Northstar Bank’s medical plan.
Compass Professional Health Services acts as your personal
advisor and advocate concerning health care matters.
Compass Health Pros can answer questions about your
health care including: doctor recommendations, inquiries
about your health and dental insurance, pricing estimates
for medical procedures, medications and other health and
dental services, reviewing of complex medical bills and
helping you understand your Explanation of Benefits.
Paid Time Off Programs
Carlile/Northstar Bank recognizes the importance of time
away from work for different reasons and provides our
employees with paid time-off programs such as vacation,
sick, personal and holidays.
401(k) Plan
Eligible after 6 Months of Service
To retire with financial security and independence is a goal
we all likely share. Carlile/Northstar Bank understands
the importance of planning for your future and offers a
401(k) plan. To help you with your financial goals, Carlile/
Northstar Bank will match your contributions 100% on
the first 3% and 50% on the next 2% of compensation
deferred. Administration of this plan is provided by John
Hancock.
Charitable Giving Matching Program
Carlile/Northstar Bank matches, dollar-for-dollar, your
donations to qualified organizations, from $50 to $500 per
calendar year.
Continuing Education
Carlile/Northstar Bank provides a Continuing Education
program that assists employees in their career development
within the Bank. Carlile/Northstar Bank will reimburse you
up to a maximum amount of $1500 per calendar year for
tuition, when you successfully complete approved courses.
