$20 Registration per day ($10 per additional sibling)
We will play with friends, make arts & crafts, have outside time, music & movement, snack (we provide), Bible
story & activity time & lunch (you provide). We look forward to having a wonderful day with your child!
Qualified, trained, loving & experienced staff will take care of your children.
Return this registration form, the additional FBC Participant Form available on the church website, the church
office or weekday office, along with the fee(s) to the FBC Church office during regular office hours: Mon.-Thurs.
8:30-5:00, and Fri. 8:30-4:30. Forms may also be returned to the Weekday Office during their office hours (M-Th
8:30am-1:30pm through May 17). Space is limited and enrollment is on a first come first served basis. Your
reservation will be confirmed once we receive the necessary paperwork and fees. You may choose to be added to
a wait list if necessary. Please make your check payable to FBC Rome. All Fees are non-refundable.
