2015 MAINE CHILDREN’S ALLIANCE GIRAFFE AWARD NOMINATION FORM INTRODUCTION Welcome! This is the nomination form for the 2015 Maine Children’s Alliance Giraffe Award. All nominations must be submitted no later than May 15, 2015. Only one nomination form can be submitted for one individual, group or business. Boards, staff, and volunteers are encouraged to work together to best describe the nominee’s accomplishments. Questions? Contact Bonnie Colfer at 207-623-1868, ext. 201, or at [email protected] UNDERSTANDING SPECIFIC CRITERIA Since 1996, MCA has been recognizing those who “stick out their necks for kids” in communities all over Maine. The annual awards program has been so successful in calling attention to the achievements of dedicated volunteers, professionals and organizations that MCA has now adopted the giraffe as its symbol, and an inspiration for its ongoing work. The Giraffe Awards bring recognition to dozens of hardworking individuals and groups in all parts of Maine. The Maine Children’s Alliance Giraffe Award is given to a Maine individual, organization or business that works, professionally or as a volunteer, in the field of education, social services or health care, or with other community organizations or institutions. Nominees will have accomplished one or more of the following: 1) Demonstrated exemplary leadership in child advocacy by representing the interests of children and their families across one or more policy issues 2) Provided or directed exemplary services and support to individual children and/or their families, including volunteer work, 3) Successfully organized advocacy for children and families at the State, local or regional level.
