coverage amounts:
• Expanded Basic Term Life: Up to $40,000
• Spouse Term Life: Up to $10,000
• Child Term Life: Up to $10,000
Other enrollment opportunities:
• During annual enrollment
• Within 60 days of a qualifying event –
marriage, birth or adoption
You may reduce or cancel coverage at any time throughout the year. You may also
update your beneficiary any time throughout the year.
Evidence of Insurability (EOI) will be required for any amounts exceeding these
guaranteed limits or if coverage is applied for outside of your initial 60-day

Child Term Life coverage terminates at age 26, unless disabled. You must
notify Minnesota Life when your child reaches age 26.

Beyond paying a benefit in the event of your
death, your group life insurance plan has other
important features:
• Additional financial protection if death or
dismemberment is due to a covered accident –
Accidental Death and Dismemberment (AD&D)
insurance provides an added level of protection
whether the covered accidental death or
dismemberment occurs at work or elsewhere.
• Take your coverage with you – If you are
no longer eligible for coverage as an active
employee, you may port your group life
insurance coverage (portable coverage ends at
age 70) or you may convert your life coverage to
an individual life insurance policy (if you retire,
you are eligible to convert up to 50 percent of
your life insurance coverage). Premiums may be
higher than those paid by active employees.
• Early benefit payments if diagnosed as
terminally ill – If an insured person becomes
terminally ill with a life expectancy of 12 months
or less, he/she may request early payment of up
to 100 percent of the life insurance amount.
