The Utility will supply Publishers with the names, addresses, telephone numbers and primary advertising

classifications (if any) of the Utility's telephone subscribers for locations requested by Publisher and updates to
this information.
B.
Subscriber Listing Information (SLI) and Updated Subscriber Listing Information (Updated SLI) shall not include
information for subscribers whose telephone numbers are unlisted or unpublished.
C. Subscriber Listing Information and Updated Subscriber Listing Information are available by Utility Directory
Name, Exchange and NPA/NXX, customer segmentation (e.g., business, residential) and for Updated SLI, by
service order type (e.g., installation, change).
D. For distribution purposes only, service addresses are available as a separate chargeable option with the SLI
Basefile. Service addresses can be provided with Updated SLI as an option to customers at no extra charge.*

Updates are also available through Internet access.
2.
Updates will be provided on a daily, weekly or monthly basis as elected by the Publisher.
3.
Orders for listing information must be submitted in writing (facsimile acceptable) on the Utility order form.
Publisher must allow thirty (30) days processing time in advance of the desired date for the information.
4.
Requests must specify the date the SLI should be pulled from the directory listings database, and for Updates,
the begin and end dates information should be provisioned. Also to be specified are the directory names,
exchanges, NPA/NXXs, listing types desired, the medium for provision of the Listing Information, and the
address for delivery.
5.
The Utility will invoice Publisher subsequent to provision of the listing information. Publisher agrees to pay the
invoice within 45 days of receipt.
6.
SLI and Updated SLI are provided solely for the purpose of publishing directories. Publisher agrees not to
distribute or sell the listing information to any third party, and agrees not to sublicense or assign any right
acquired by Publisher under this tariff. No right or license is granted or implied under any proprietary rights of
the Utility or affiliates that subsist in portions of directories distributed by or for the Utility other than SLI and
Updates. Publisher certifies that the listing information shall be used only for the purpose of publishing
directories.

SPECIAL CONDITIONS
1.
This offering authorizes a publisher, as described herein, to reproduce names, addresses and telephone numbers of this
Utility's customers contained in Utility's telephone directories, including Directory Assistance Directories as hereinafter
described. Such reproduction rights do not include names, addresses, telephone numbers, art work, headings and other
materials contained in this Utility's classified "yellow page" directories and directory sections or other utilitys' customers listed
in this Utility's directories.
2.
Customer Telephone Directories are generally published annually. Directory Assistance (monthly reprint) Directories and
published monthly except there may be a slight deviation in the month in which the corresponding regular Customer
Telephone Directory is published. Customer Telephone Directories and Directory Assistance Directories will be available
within ten days following publication of each directory.

This offering is limited to publishers engaged in the business of publishing a general directory for general public use and
distribution.
A general directory is defined as a publication normally distributed periodically to the public which contains a classified
compilation of a number of individuals or concerns together with their respective telephone numbers and advertisements
regarding their products or services. These directories may also contain an associated compilation of telephone customers
names, addresses and telephone numbers.
4.
The publisher shall not permit anyone but employees of publisher or authorized representatives to inspect, use or reproduce
any information furnished hereunder. Reproduction rights under this offering are nonassignable and nontransferable.
5.
The Utility assumes no responsibility or liability for any error in the information furnished. The publisher shall indemnify the
Utility and hold it free and harmless of and from any and all claims, demands or damages that shall or may arise from the use
of this offering.
6.
The Utility reserves the right to discontinue any of its directories in whole or part, to rearrange or change any such directory in
whole or in part or to split or combine one or more of such directories as it deems necessary in the ordinary conduct of its
business.

The Utility's liability shall be limited to direct and actual damages which shall not exceed the fees paid by
Publisher for the information provided by the Utility. Under no circumstances shall the Utility be responsible or
liable for indirect, incidental, special or consequential damages, including, but not limited to, economic loss or
lost business or profits.

Listing Information is supplied as is. The Utility makes no warranty of any kind, express or implied, including
without limitation warranties of merchantability or fitness for a particular purpose.
9.
Publisher agrees to indemnify, defend, and hold harmless the Utility, its affiliates, officers, agents, and
contractors (collectively, the "Indemnified Parties") from all losses, claims, demands, damages, expenses,
suits, or other actions, or any liability whatsoever, including, but not limited to, costs and attorney's fees,
suffered, made, instituted, or asserted by any third party arising from a Publisher's use of the listing information
provided as requested.

7.
Charges shown above for this offering include furnishing a single current copy of the directory requested and such directories
are not subject to return to the Utility.
8.
The publisher shall furnish a copy of any published directory containing any listing information furnished hereunder to the
Utility within 10 days following publication. The Utility may refuse to furnish reproduction rights thereafter to any publisher
who fails to comply with this provision.
9.
The charges shown above are based, in part, on the estimated number of listings the publisher expects to include in one or
more directories. Charges for estimated number of listings are payable in advance or the Utility may, at its discretion, accept
a guarantor to secure payment. The Utility, within 40 days following publication of one or more such general directories,
shall:
a.
refund or bill, as appropriate, any difference between the estimated amount collected and the applicable charge or,
b.
where a guarantor has been accepted, determine the applicable charge, advise the guarantor of such applicable charge
and bill the publisher that amount.
10.
Each listing or line of information, included in a publisher's general directory will be counted to determine the charge for each
1,000 listings or fractions thereof. This charge applies separately to each separate general directory containing any listing
information furnished by the Utility. The charge shown above for reproduction rights, each directory, each issue, applies only
once to each Utility directory issued.
11.
The estimated charge for listings to be included in a publisher's general directory, collected or incurred hereunder, is
refundable or subject to cancellation, upon request, in the event a publisher fails to produce a general directory within six
months from date of purchase of reproduction rights hereunder. The charge for reproduction rights, each directory, each
issue, is not refundable or subject to cancellation.
12.
Publisher shall not make any representation to the public, prospective advertisers or others, expressed or implied, written or
oral, to the effect that Publisher (and its publications) is the same as, apart of, or associated with the Utility or Utility's
Directory Company.
