Please complete the forms and mail/return them to the attention of your Head

You will not be allowed to participate in any sport until this
information is completed and returned to the NIC Athletics Office.
North Idaho College will not be responsible for payment of any medical injury
claims if this information is not complete and on file in the Athletics Office at the
time of the injury.
