Meet ALEX® for help with all of your benefit needs
Picking the right benefit plans can be a challenge. Which medical plan is best for me? How much should I save in my
flexible spending accounts? Should I get extra life insurance? Does a health savings account make sense for me?
The decisions are important, and a lot goes into making the right choice. To make the process easier for you,
Old National has brought in an easy-to-use online tool called ALEX.
How ALEX works is simple.
All you have to do is log on and respond to ALEX’s questions. ALEX will prompt you for some
basic information about you and your family, ask a few questions about how your personal
situation (everything you say remains confidential, of course*), and help you figure out what
to choose based on your responses.
Talking with ALEX feels like having a conversation with a real person, and because ALEX uses
simple language and avoids insurance jargon, its explanations and recommendations are easy to
understand. Also, because ALEX is available from any computer with an internet connection, you can
use it with your family as you consider your options. And if you have any questions about how
anything works, ALEX can walk you through them.

Benefits Open Enrollment: November 7-21 (portal closes at 5pm CST)
Who is Eligible:
All Full-Time and Part-Time Level 1 associates hired prior to November 2, 2016.
Where to enroll: Enroll online through the MyONB Portal or by calling the ADP Benefits Solution Center at 1-800-240-7155.
• Benefits will be effective January 1, 2017.
• Visit with ALEX your virtual benefits enrollment counselor at myalex.com/oldnational/2017.
• Detailed information regarding available plans and rates can be accessed at oldnational.com/associates.
Eligible associates should consider the following when making their benefits elections for the 2017 plan year:
• Associates currently enrolled in a Health Care Flexible Spending Account Plan or Dependent Care Flexible Spending
Account will NOT be automatically re-enrolled in these plans for the 2017 plan year. Associates wishing to have these
benefits must re-enroll.
• Health Savings Account (HSA) participants will also need to elect their 2017 contribution amount in the open enrollment portal. Any current year contribution elections do not rollover. Participants may increase, decrease or stop their pre-tax HSA contributions during the plan year.
Associates’ actual enrollment elections for the 2017 plan year will reflect the final options submitted on the MyONB portal as
of the close of the Open Enrollment period.

Castlight, an online service newly offered in 2017 as a part of your medical plan, solves that issue

Use LiveHealth Online to get quick, convenient access to care
With LiveHealth Online, you have a doctor by your side 24/7. LiveHealth Online lets you
talk face-to-face with a doctor through your mobile device or a computer with a webcam.
No appointments, no driving and no waiting at an urgent care center.
When you need to see a doctor, simply go to livehealthonline.com or access the LiveHealth
Online mobile app. Select the state you are located in and answer a few questions.
Use LiveHealth Online for common health concerns like colds, the flu, fevers, rashes,
infections, allergies and more!
Establishing an account allows you to securely store your personal information. Plus, you can
easily connect with doctors in the future, share your health history and schedule online visits
at times that fit your schedule.
Once connected, you can talk and interact with the doctor as if you were in a private exam room.
LiveHealth Online is available to all associates and dependents covered under an
Old National medical plan.
• PPO participants - cost is the same as an office copay
• HDHP participants - cost is $49 per call until deductible is met
Activate Health and Wellness Centers (Medical Clinic Access)
Old National is excited to be partnering with Activate Healthcare, to provide access to Health and
Wellness Centers throughout our footprint. Activate currently operates 16 locations throughout
Indiana and Wisconsin with plans for continued expansion throughout our footprint.
Activate’s facilities provide associates and dependents (over the age of 3) who participate under an
Old National medical plan access to convenient comprehensive primary and acute healthcare and
wellness services delivered by experienced physicians and nurse practitioners.
Activate provided services include:
• Primary/Acute Healthcare – Illnesses, condition management (asthma, diabetes, etc.), lab/blood
work, allergies, and other medical needs for which you’d normally consult a doctor or medical clinic
• Preventive Care - Physicals, flu shots, health coaching, health risk assessments
•Pharmaceuticals – Generic prescription medications dispensed on site
Activate’s provided services are:
•Affordable: Services are free of charge for PPO medical plan participants. Due to IRS rules, High Deductible Health Plan
(HDHP) participants will incur a small cost per visit ($20/visit and $5/filled prescription) for non-preventative services
until plan deductibles are met.
•Convenient: More time with the doctor and less time waiting
Activate Health and Wellness Centers are open five days a week (hours as early as 6am and as late as 7pm)
to meet you and your dependents health and wellness related needs.

Benefit plan design changes for 2017

Due to Health Care Reform and the pending Cadillac Tax,
the PPO 250 plan will no longer be offered beginning 2017.
Participants who are currently enrolled in this plan, who make
no enrollment changes, will be moved into the PPO 850 plan
for 2017.
•
The Emergency Room copay for all PPO plans will be changed
from $200 to $250 per visit.
•
The pharmacy copays will be consistent across all medical plans with the exception of the High Deductible Health Plan
(HDHP) 2000. See Medical Plan Summary for more details.
•
The vision frame allowance for the vision plan has been
increased from $120 to $130.
Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA)
FSA account maximums
The maximum contribution allowed for FSA medical elections under the ONB Flex Plan has been indexed to the IRS
allowed amounts. For 2017, the maximum annual contribution to the Health Care FSA will be $2,600. The maximum
contribution for the Dependent Care flexible spending accounts will remain at $5,000.
ONB contribution for HSA
Eligible associates enrolled in a High Deductible Health Plan (HDHP) with an Old National
HSA effective January 1 will receive an ONB company contribution of $400 for single coverage
and $800 with any other level of coverage. Please note the ONB contribution counts towards
your 2017 maximum IRS HSA contribution limit.

The annual maximum allowable contributions to an HSA, as established by the IRS,
for 2017 are:
• Individual: $3,400
• Family: $6,750
Individuals 55 and older can make an additional catch-up contribution of $1,000 in 2017. Associates enrolling in a
High Deductible Health Plan for the first time during open enrollment must submit an Old National (HSA) application by
December 2, 2016, in order to avoid delays in receiving the company’s contribution, their personal contribution and/or
the HSA debit card. No application is required by associates who already have an Old National HSA.
If you’re enrolled in a HDHP and the Health Care FSA, you can only use your Health Care FSA to pay for eligible dental
and vision expenses (Limited Purpose FSA).
Associates currently enrolled in the FSA or HSA plans will NOT be automatically re-enrolled for the 2017 plan year.
Associates wishing to have these benefits must re-enroll.
