We will attempt to provide you with an honest evaluation of the temperament and health of
any dog we foster out. Often a complete history is unavailable and unknown problems or
concerns may be discovered while in foster care. Are you willing to accept and work with the

approval for fostering, information will be verified.
I certify that I am physically and financially able to care for the routine care of this dog. FBFC
will cover medical and other special purchase items for this dog but some payments may have
to be paid up front and submitted for reimbursement.
I give my veterinary clinic permission to release to FBFC Executive any requested documents on
pets they have attended to for me in the previous 3 years and this authorization is current for
one year from application to foster
I understand that by signing this form, I agree to release and covenant to hold harmless The
French Bulldog Fanciers of Canada and its members from any claims, damages, costs, or actions
incurred as a result of the care or actions of the animal. I accept full responsibility for the
animal(s) actions at all times, and release the FBFC from any liabilities or damages that may be
incurred as a result of fostering such animal(s).
