The completion of fiscal year 2014-2015 has just occurred. What an eventful year!
Besides our staff delivering quality services and care to our clients, there have been many
significant events and successes. Here are just a few:
-Acquisition of the Hartzel Fan Building for JRDS Industries;
-JRDS was a finalist for the Chamber’s Not-for-Profit Organization of the Year and
Board member Janet Bantz was a finalist for the Citizen of the Year;
-Opening of a newly remodeled one-story home;
-Remodeling client bathrooms at two homes;
-Updating of the Main Street Home due to drainage;
-Upgrading lavatories at the JC Center;
-JRDS kicked off its new monthly JRDS Gives Back to Randolph and Jay County;
-Conducted “Be Safe” training that demonstrates how to interact with a police
officer if approached; it involved 26 law enforcement officers from Jay and
Randolph counties in IN and Mercer County OH;
-With the help of the United Way of Jay County, we obtained a Hoyer lift for
transferring clients and some new wheelchairs;
-A number of staff had the privilege of playing golf alongside two of our clients at
the Jay County Chamber golf outing and the golf outing to support LifeStreams.
On behalf of the clients, staff and the Board of Directors, we offer you our thanks for your
interest in JRDS and you support with its past and future success. Without your continued
support, carrying out the JRDS mission would be a lot more difficult.

Personal Successes
Since transferring in February to BLS to better accommodate his needs, Stephen Moore has succeeded in meeting his
goals! When in the community his interactions with others have improved—he uses more eye contact and doesn’t turn
away when he speaks to people. But as much as he likes to go in to the community, he always makes sure he is
back for break time. Stephen chooses his own activities and converses with peers. He joins group activities and often
reminds peers and staff what they should or should not be doing. Stephen has shown great progress since attending
the BLS program.
Ryan Wiggins has been with JRDS for 16 years and in that time has proved he can be counted on to lend a hand. He
is dependable and willing to work at any job and the quality and quantity of his work is exceptional. He is able to
learn a job easily and often can show you a better way to do it! He has helped staff and his fellow consumers learn
to communicate with American Sign Language. A wonderful artist who creates whatever is needed, he is a friendly,
smart and amazing person who continues to show progress. We are grateful to have him as part of our team.
Lee Shockney entered our agency in 1999. He was a very shy individual who often looked down at the floor when
you interacted with him. If you asked him a question he usually responded with either a nod yes or no but wouldn’t
make eye contact. He has shown tremendous growth throughout the years. When he goes into the community to
the local library to check out the newest Power Rangers or Pokemon game characters he asks the librarian to use a
computer and thanks her when she returns his library card. Recently, he began reading books related to his games
and/or TV shows and enjoys telling staff all about the characters and their journeys. Today, this quiet young man will
make eye contact with others and chats all the way to the library and back. We are proud of his accomplishments.
For over 20 years Steve Hill has volunteered at Willard Elementary School. Through his volunteer efforts, he has
grown socially. Instead of relying on someone to talk for him or help him with his job, he walks into the school and gets
straight to work. The staff and students at the school enjoy having him volunteer and when he is not there they always
say that they missed seeing him. Watching him interact with the students and staff is a real delight. We are all proud
of Steve’s accomplishments.
Aaron Rockwell graduated from Jay County High School in 2015. Throughout high school he was employed with
Marsh Supermarket in Portland where you may see him bagging your groceries or collecting carts in the parking lot.
After graduating from high school, he has continued to work with Marsh in Portland and picked up a second job at
JRDS in the janitorial area. While completing his task he is very courteous and polite to staff and consumers. All
reports show he is doing a fabulous job. Aaron is not positive on what he would like to pursue in the future though he
has shown a lot of interest in auto mechanics. We are sure whatever path he decides to take in the future he will
give it his best and be successful.
Lisa Page is a person who has persevered throughout her life. Even though Lisa has had many struggles and
hardships, she has always made family, friends and staff feel like they were the most important person in the room.
No matter how she feels, she has a smile on her face and no matter how bad the staff feels she can put a smile on
their face. Lisa has transitioned from Main Street to Meridian I where she has flourished. Lisa has formed positive
relationships with her two new housemates who she treats like sisters. She has always wanted to help and she is
eager and willing to do whatever the staff or her housemates ask her to do. Lisa asks her staff if she can do chores
just because she knows they need to be done. Lisa is willing and excited to work on her goals everyday. We are
happy to have Lisa as part of the JRDS family.
Richard Perkins is a pleasure to take out and he always has a smile. He jokes with everyone and loves to tease but
he also has a very tender heart. He often shows his tender side by purchasing cards for fellow nursing facility
residents. Richard loves to go to flea markets to look for collectibles, especially Beanie Babies and Coca-Cola
collectibles. He loves nature and keeps it close by growing plants in his room. He really enjoys attending Ag days in
Randolph County and the Tri State Antique Gas Engine & Tractor show in Jay County. We are glad that Richard is
part of the OBRA program.
In 2003 Melinda Codling was quiet and a bit timid. She has always liked the song “It’s My Life” by Bon Jovi because
she strives to be an independent woman. Over the years she’s become willing to work to improve the skills needed
to become more independent and has learned to be responsible for her own happiness! There have been struggles
along the way, but she always overcomes them. She recently lost a family member yet decided to celebrate that
person’s life and cherish the good memories. She also had to pack up all of her belongings to move to a new home.
She takes pride in her new room while the move taught her that she doesn’t need as many belongings to be happy!
Melinda has become a strong, confident, assertive, and courageous woman. This year, she even decided to go to
camp by herself and was excited to be on her own and have the opportunity to make new friends. She has more control of how she
reacts to her feelings and is more accepting of other’s feelings. Like the Bon Jovi song, Melinda feels that, “It’s my life, It’s now or never, I
ain’t gonna live forever, I just want to live while I’m alive, It’s my life.”
Services Summary
Community Employment Follow Along - One consumer is involved in a community employment
follow along services. This person receives up to five hours of one-on-one support each month to
ensure that he is doing his job correctly and meeting the expectations of his employer.
Day Habilitation - JRDS provided goal-oriented training in the Jay and Randolph county facilities as
well as the community. This training empowers recipients to develop and maintain independence in daily
living skills. Each individual had the opportunity to learn functional skills in facility and/or natural
settings. In Day Programming services based in the two facilities, thirty-seven consumers are served by
seven part-time and two full-time staff. In Day Programming services at Jay, Randolph and Delaware
county nursing facilities, two part-time, one full-time and one substitute provide community based
services for fourteen persons with developmental disabilities.
Life Skills - JRDS provided twenty-three consumers with instruction to acquire, increase or maintain
independent functions of daily living in their communities. Activities are designed to empower individuals
in exercising more control over their lives and enhancing their participation in and connection with the
community where they reside. Many of these consumers volunteer at community sites. One full-time
and three part-time staff oversee these services.
Residential - JRDS provided supervised group living services for twenty-four adults in four accessible
group home settings located in the towns of both Winchester & Portland. Residential programs
employed thirty-one full, part-time and substitute staff who provided administrative functions as well as
direct care services. These services focused on a continuum of self-sufficiency and community
integration opportunities.
Waiver Community-Based Supported Living Services - Forty-nine individuals with special needs
were provided services in their homes, their parents’ home or in other community settings. These
individuals received direct training and/or assistance in the areas of self-care, mobility, life skills,
education, social skills, behavior management and community integration. The objective of habilitation
services is to empower a person to maintain and/or enhance their level of independence. These state
and federally funded services are provided by six full-time and twenty-one part-time/sub direct service
staff and three support/supervisory staff members.
Waiver Home Services - JRDS manages eight homes that provide 24/7 services to twenty individuals.
Twenty-one full-time and twenty part-time/sub staff provide in-home services that include participation in
community activities at their individual comfort level, general self-help skills, socialization activities at
home and in the community and any other activities in their areas of interest. Waiver services utilized six
Behavioral Consultants, thirteen Case Managers and one Music Therapist.
Prevocational - Fifty-nine consumers are involved in prevocational services where they receive
marketable work skills in the two facilities as well as in community-based work training opportunities.
These work opportunities offer training to assist persons with disabilities in gaining the skills needed
should they elect to join the community work force. Contract work in-house and at enclaves is completed
for industries including, but not limited to, Anheuser-Busch/Jay County Beverages, JRDS Industries,
Sonoco Protective Solutions, Randolph Farms, MSSL, and Silver Towne.
JRDS Industries
As part of this mission, JRDS has long held relationships with area industries in
both Jay and Randolph Counties. These relationships provide work training and
income opportunities to clients who have the ability and desire to participate in the
contract work. JRDS clients have performed contract work for various local
industries for over three decades including, but not limited to, Randolph Farms,
MSSL, Applegate, Sonoco Protective Solutions, and Hartzell Fan.
Over the past few years, much effort has been given to expanding the contract
customer base with the goals of providing an integrated work environment and
diversifying JRDS revenue streams. Leaders within the sate government have
been quite vocal with the provider community over the past few years about the fact that
reimbursements for services will not keep up with rising costs of service delivery and that providers like
JRDS must develop other funding streams to continue to operate.
So, in the fall of 2014, JRDS made the strategic decision to expand
the industrial services into the former Hartzell Fan building after
much study and discussion.
Currently, a crew of up to eight clients complete work for Randolph
Farms as needed; a crew of up to eight clients complete work for
MSSL; and up to seventeen clients from two counties combine to
create the crew at the JRDS Industries building. Industries was
opened to create new income and training opportunities for the
clients and to increase the non-government revenue in order to
sustain JRDS services for many years into the future.

JRDS?
Staff
This is the most rewarding job in the whole world; makes me feel like I’m part of something greater.
Working at JRDS is the most fulfilling job I have ever had. I never dread coming to work because I
know that my clients/families appreciate me and I know that I bring joy to the clients that I have
had or do have the pleasure of working with. This is definitely the place I see myself retiring
from.
JRDS Annual Fund Donors
JRDS seeks every possible avenue of government and foundation support to fund its programs. While this income
presently provides the majority of funds that are needed, it cannot cover all expenses and operating costs.
We at JRDS give a special note of thanks to those who financially support our mission. We acknowledge that many
of the day-to-day needs and the important “extras” that help our clients achieve their fullest possible potential would
likely go unmet if it were not for your generosity.
