Dear Brother Simpson:
The Teamsters Disaster Relief Fund recently received a "Disaster Ale'1 Notice"
for the state of Illinois regarding the recent stonn~'I winds and tornadoes that caused
major damage in the following counties on November 17,2013:
Champaign, Douglas; Fayette, Grundy, Jasper, La Salle, Massac
Pope, Tazewell, Vermilion, Wabash, Washingtoo, WaYDe, Will and Woodford
On behalf of Genera] President lioffa, we would like to request your a..C)sistance to
aid Teamster members in your area that have suffered a loss. We recognize the
devastation in your area and!! although our funds are limited, would like to help as many
people as possible. Enclosed are the ~'Notice to Post" and the "Request for Ht~]p'~ form.
Please inform us if any of your members have been impacted by the disa~;ter. The
completed "Request for Help" fonns (including all requested documentation) must be
returned by Friday, Jannary 10,2014.
. While the immediate crisis is over, the IBT realizes the difficult task of rebuilding
lives and the need to do an outreach to agencies that can assist. Please advise our ol1ice
at (202) 624-8971 of any needs you may have, and we will do our best tn accommodate
them_

Illinois Severe Storms,. Straight-line Winds and Tornadoe~'
The Federal Emergency Management Agency (FEMA) has declared a major disaster for
the state of Illinois opening the way for the use of federal disaster funds tor people affected
by the severe storms, straight-line winds and tornadoes that occurred on November 17,
2013. Assistance is available to individuals and households in the following counties who
may have experienced a loss due to this disaster:
Champaign, Douglas, Fayette, Grundy, Jasper ~ La Salle, Massac,
Pope, Tazewell, Vermilion, Wabash, Washington. Wayne, Will and Woodford
Teamster members, please contact your Local Union office ifyou have sulfered losses due
to this storm; fill out a "Request/or Help" form (;nclud~ all requested docunJtpttation) if
you would like to request a grant from the Teamsters Disaster ReliefFund.
DISASTER RELIEF FORMS MUST BE RETURNED l~O THE
