As you may be aware, HealthyCT, a newly formed non-profit, private commercial health
plan, was awarded operational startup and solvency funding in June of 2012. This
significant accomplishment was due in large to the generous support and sponsorship of
the Connecticut State Medical Society and the Connecticut State Medical Society IPA (CSMSIPA). On behalf of the board and management of HealthyCT, we would like to thank all
those who have helped support us. Without their assistance we would not be here today!
HealthyCT is referred to under federal healthcare reform as a “CO-OP” which stands for
Consumer Operated and Oriented Plan. Our vision is to develop a cutting-edge health plan
that facilitates the delivery of high-quality, patient-centered medical care with strong
physician-patient relationships at its foundation.
HealthyCT has partnered with the CSMS-IPA to help develop our provider network and we
are reaching out to you with an invitation to participate. For your reference, we have
attached a document that provides a market sampled fee schedule, approved by the
independent members of our board, for the relevant, highly utilized CPT codes for your
specialty. Additional fees for codes not provided on the enclosed schedule will be available
and accessible to providers by accessing the home page of the HealthyCT web site at
www.healthyct.org.

in influencing change in the way health insurance has been traditionally delivered. If you
