Development of an effective mechanism for application of that proactive detection and management of deceased organ donors based on directory of regulation of organ donation & transplantation in kingdom of Saudi Arabia & Saudi national protocol for diagnosis of death by neurological criteria Application of quality assurance system and ensuring quality assessment and performance improvement of the proactive detection of deceased organ donors program in kingdom of Saudi Arabia through regular assessment of the benchmark or level of acceptable performance. About the Application Crucial tool for online proactive detection and management of deceased organ donors which allow considerable data base collection for each deceased donor detected from each Donor Hospital concerned by the program. This data base will be recorded, archived and analyzed retrospectively under complete security and confidentiality creating a data base registry which allows auditing and detecting areas of improvement. This data base registry can be used to provide an online dashboard of 3 levels for the executive Users, regional Users and hospital users. The application consists of 4 stages representing the critical pathway for deceased organ donation Possible DBD →potential DBD →Eligible DBD →Actual DBD →Utilized DBD 1 Proceeding throughout the whole cascade of the critical pathway by completing these stages will figure out areas for improvement where each stage has a defined target or benchmark through measurable objectives. This page includes the following data: Hospital information Possible deceased organ donor information Possible deceased organ donor recognition criteria Cause of death Past medical history Sudden cardiac arrest - It is the responsibility of the Hospital Donor Medical Coordinator (H-DMC) or any delegated ICU staff to complete this page within the appropriate time frame i.e. as soon as a possible deceased donor is detected and this can be done during routine daily rounds or any other time during the day. - Completion of this page means fulfilling the definition of possible deceased organ donor - Possible deceased organ donor recognition criteria and field with asterisks (*) has to be fulfilled in order to proceed to next step - Proceeding to next page will trigger the 1st notification sms alert - The 1ST notification sms alert means fulfilling possible deceased donor definition and transition to potential deceased donor with consequent responsibilities to the concerned parties. The 1st notification sms alert will be sent to: 1. The hospital information: Hospital name and reporting date will be automatically generated by the system according to the user level and the default date of entry Hospital Department will be down list with capability to choose others if not in options listed then to specify which department Other information such as ICU Bed, Treating Physician and telephone numbers will be manually entered by the user. 2. Possible deceased organ donor information: Nationality will be down list with capability to choose others if not in options listed then to specify which nationality. Blood group and Gender will be down list Dates like birth date, date of admission and date of ventilation will be down listed calendar box which will allow you to choose the requested date. Possible Deceased Organ Donor Recognition Criteria All the criteria has to be entered by user The status will be automatically generated by the system (no Entry from the user) 5. Sudden cardiac arrest All fields has to be entered by the user 9 PROACTIVE DETECTION OF DECEASED ORGAN DONOR MAXIMIZING COOPERATION BETWEEN SAUDI CENTER FOR ORGAN TRANSPLANTATION AND INTENSIVE CARE UNITS (MCIS) Potential donor after brain death (Potential DBD) General Guidance: - This page represents the 2nd step in application of the critical pathway for deceased donation Potential DBD Donor: A person whose clinical condition is suspected to fulfill brain death criteria. - This page includes the following data: First clinical examination for diagnosis of death by neurological criteria. Confirmatory tests for diagnosis of death by neurological criteria. Second clinical examination for diagnosis of death by neurological criteria. SCOT flow chart. - It is the responsibility of the H-DMC or any delegated ICU staff to complete this page within the appropriate time frame and this can be done during routine daily rounds or any other time during the day (completing diagnosis of death by neurological criteria according to Saudi national protocol) with optional choice to complete SCOT flow chart and shared responsibility with RCODMC to complete this chart. - Completion of this page means fulfilling the definition of potential DBD donor. - First clinical examination step has to be completed in order to proceed to the confirmatory tests then positive result of any of the confirmatory ( consistent with brain death diagnosis ) has to be in order to proceed to the 2nd clinical examination then completed 2nd clinical examination is mandatory to proceed to Apnea test. - Finally, Positive Apnea test (consistent with brain death diagnosis) is mandatory in order to proceed to next step - Proceeding to next page will trigger the 2st notification sms alert - The 2nd notification sms alert means fulfilling potential deceased donor definition and transition to eligible deceased donor with consequent responsibilities to the concerned parties. - The 2nd notification sms alert will be sent to: 1. Breaking bad news. Notification to RCO. SCOT flow chart. - It is the responsibility of the H-DMC or any delegated ICU staff to complete this page within the appropriate time frame and this can be done during routine daily rounds or any other time during the day (ensure breaking bad news to family members and complete understanding of the concept of brain death) with optional choice to complete SCOT flow chart and shared responsibility with RCO-DMC to complete this chart. With Regards to Medical suitability, it will be determined after consultation with SCOT – DMC. - Completion of this page means fulfilling the definition of Eligible DBD. - Breaking bad news to family members is mandatory in order to proceed to next step. - Proceeding to next page will trigger the 3rd notification sms alert. - The 3rd notification sms alert means fulfilling eligible deceased donor definition and transition to Actual deceased donor with consequent responsibilities to the concerned parties. - The 3rd notification sms alert will be sent to: 1. Medical suitability for the donor : The user will optionally define if the donor is suitable or not suitable after consultation with SCOT-DMC If not suitable then user has to fill reason for non suitability for donation. 2. Breaking Bad News : The user will define if the family of deceased is informed about death of their relative or not. If yes ( family informed about death of their relative) then - User has to fill date and time when informed. - User has to fill the Conveyed by box (A) by typing name of the physician who conveyed the message to the family. If No (family not informed about death of their relative) then user has to mention honestly the reason for not informing the family for establishing quality assessment and performance improvement. A 15 3. Notification to RCO/SCOT : The user will define if notification to RCO/SCOT DMC done or not. If yes (RCO/SCOT-DMC notified about deceased donor status) then - User has to fill date and time when notified. - User has to fill the Name of RCO/SCOT-DMC who has been notified. If No (RCO/SCOT-DMC not notified about deceased donor status) then user has to mention honestly the reason for not notifying for establishing quality assessment and performance improvement. Recovery procedure. - It is the responsibility of the H-DMC or any delegated ICU staff to complete this page within the appropriate time frame and this can be done during routine daily rounds or any other time during the day with shared responsibility with RCO-DMC and SCOT-DMC to complete the Recovery procedure step. - Completion of this page means fulfilling the definition of Actual DBD. - Family Approach step has to be completed with presence of written consent in order to proceed to Recovery procedure step. - Presence of written consent will trigger the 4th notification sms alert. - The 4th notification sms alert means presence of consented eligible donor with consequent responsibilities to the concerned parties to facilitate the recovery procedure of organs for purpose of organ donation. - The 4th notification sms alert will be sent to: 1. RCO- DMC/ DAC: to head over to the hospital and follow up the donor with the H-DMC & ICU Staff in addition to coordination with SCOT-DMC to facilitate for the recovery procedure of organs for purpose of organ donation. 2. SCOT - DMC: to follow up with RCO-DMC & H-DMC as well as coordinate with transplant centers for allocation of donated organs and follow the organ allocation plan. 3. SCOT - DAC: to follow up with RCO-DAC & SCOT-DMC to facilitate all logistics and administrative issues which may hinder recovery procedure of donated organs. 4.
