This project aims to reduce the number of people dying or disabled
by AF related stroke by early review and detection of AF and
optimising the use of anticoagulants in line with NICE CG180
guidelines.
Across Kent, Surrey & Sussex (KSS) there are approximately 6000
strokes per year. One in three is caused by atrial fibrillation (AF)
(SSNAP 2014/15). Anti-coagulation, can significantly reduce the
risk of stroke in patients with AF. Despite guidance, data indicates
that anticoagulation is still widely underused, or over prescribed for
some patients.
The project is to enable 3 interventions that deliver an impact at
various stages of the patient’s diagnosis and treatment:

Provide timely reviews
Increase the prevalence of AF
Increase optimal anticoagulation
Bespoke AF packages will be implemented into cluster groups of
GP practices as a ‘wave of change’.
As project sponsor, I am passionate about clinical leadership with a
broad background of improvement in the NHS. The Darzi
Fellowships offer a fantastic opportunity, to embed clinical
leadership at the heart of the delivery of STP ambitions and those
set out in the Five Year Forward View. As an AHSN we have a
central role to play in the spread and adoption of breakthrough
innovations and improving patient experience, outcomes and
delivering financial sustainability.
