Self-Review

Go to mylearningplan.com and log in using your KUSD credentials. Scroll to the bottom of the page and
you will see the following information:
1. Under My Evaluations, click on Open Teacher Self Review - Self-Review form for submission

Check to ensure that your name is spelled correctly.
Check to make sure that you correct primary building is listed.
Check to ensure that you are assigned to proper administrator.
Check to make sure that you have the correct Evaluation Type (this can be cross checked with
your administrator.)
6. If everything identified in steps 2-6 is correct, then you can move on to completing your selfreview. If anything needs to be changed, then please inform your administrator.
COMPLETING YOUR SELF REVIEW
Scroll down the page until you see the following:
KUSD has determined that each evaluated employee will complete the self-review at the beginning of
each school year. It is recommended that this process is done and submitted to the evaluator by
September 30th.
1. For each component, the employee shall reflect on their abilities and check (click on box) the
critical attributes that they believes best matches their performance (please note, that nonteacher groups may not have critical attributes and should move on to the next step.)
2. At this point, use the reflection to score your performance on the scale of 1-4. To complete this,
simply click on the information under the numbered score until it is highlighted.
3. It is optional at this point to click on the words Enter Rationale and explain to your evaluator the
reasoning behind your choice.
4. Complete the same steps for all components in the self-review.
5. After reaching the final component, you will the following options:
a. Submit: This will send your results to your evaluator and must be the final step in
completing the process.
b. Save: This will save your information so that you can come back at a later time and finish.
c. Save and notify: Allows you to save your information and notify your evaluator of your
progress and any issues that you are having finishing the process.
d. Reset: This will wipe out all progress so that you can start over.
e. Print: creates a .pdf that can be printed.
f. Comment: allows you to add statements that are shared with your evaluator. All comments
appear at the bottom of the form.
