What is a Journey Board?
Phoenix Children’s Hospital values family-centered care. This means we care for your
child as part of your family, and we tailor our care to meet your family’s needs.
We want to make sure that when you take your child home you understand your
child’s condition, how to treat it, what problems to look for, and how to respond. We
also want to make sure you know who to call if you have questions or concerns.
It is best if we don’t teach all this in the hour before you take your child home from
the hospital. We may forget to tell you something important, or you may have
questions you don’t get a chance to ask.
So, to prevent this, we give you a Journey
Board when we meet you. The Journey Board
is a tool we use to make sure you understand
everything that is important to care for your
child. Every box on the Journey Board is
something you may need to know about, such
as, “I can tell you about my child’s medicines.”
We will talk to you about your child’s care every

day. Then we will ask you to teach back to
us the items on the Journey Board. You’ll also see cues on our handouts, like, “Now
that you’ve read this, tell your nurse or doctor how you would . . .” This is not a
test. This is to make sure we explained everything you need to know in ways you
understand. If we were not clear, we will find another way to explain it. We want to
make sure you know what to do, and how to do it.
If you don’t understand something about your child’s care, ask us. If English is not
your first language, and you would understand us better if you heard it in another

language, ask for an interpreter.
If your nurse or doctor does not give you a Journey Board, ask for it. We want to
make sure you are comfortable and confident that you can take care of your child
safely at home.
