Amendment No.: (2) Two
The Informal Request for Proposal P14-005 for Automatic Teller Machine Service is hereby amended with
the following changes/clarifications:
1. The Informal Request for Proposal – P14-005 – Rev-1, Section C. UA INSTRUCTIONS TO OFFERORS,
Proposals: (a) & (b), are hereby replaced in their entirety with the following:

(a) The Offeror must submit (1) original and three (3) copies of their technical proposals (including all items indicated in the
CHECKLIST OF REQUIRED SUBMTTTALS for this Informal Request for Proposals, and one (1) original of the Price/Cost
Schedule in a separate file to the University of Alaska address designated herein for receipt of proposals.
(b) Late proposals will not be considered and will be returned to the proposer unopened. A proposal is late if it is not
delivered to the University of Alaska address listed in this IRFP on or before the time specified herein as the deadline for
receipt of proposals.
2. The Informal Request for Proposal – P14-005 – Rev-1, Section J, ADITIONAL CONTRACT PROVISIONS, #2. TERM
OF CONTRACT, is hereby replaced in its entirety with the following:
2. TERM OF CONTRACT: A contract will be awarded for the period beginning approximately July 1, 2014 - June 30, 2017,
with options to renew for three (3) additional one (1) year terms following the initial term, upon written agreement of the
parties. The parties must mutually inform each other of their intention to renew the agreement no later than (90) ninety days
prior to the initial contract terms expiration date If all renewal option years are exercised, the contract shall automatically
expire June 30, 2020.
ACKNOWLEDGMENT; Amendment #2, IRFP (P14-005), Automatic Teller Machine Service. This Amendment must
be signed and returned with your proposal or otherwise acknowledged prior to the closing date and time listed in this
amendment. If you have already submitted a proposal and need to make corrections, please submit a corrected proposal prior to
the closing. The closing date is Thursday, April 17, 2014, at 4:00 PM local Alaska time.
