On this day came Connie S.Ashley, who, after being duly sworn, did depose and
say:
{I) That 1 am an adult over the age of 18 years and am competent to make
this Affidavit;
(2)
That, as the OfTice Manager of the above named wrecker company, I
recently filed an application with the Public Service Commission of West Virginia
(Commission) to change the rates that this wrecker company charges for third-party
towing andlor stcirage;
(3)
That the third-party towing and/or storage rates proposed will be (check

the rates charged for tnwing
andlor storage of the same type vehicle when such towing is requested by the owner or
operator of the vehicle;
