This addendum forms a part of the Contract Documents and modifies the original Proposal Documents
dated December 20, 2016. Acknowledge receipt of this Addendum in the space provided on the Proposal
Form (Section 00300). Failure to do so may subject Proposer to disqualification.
Item No. 1–
Revise the address for the Old COM Senior Adult Center to “1130 Delmar” throughout documents.
Item No. 2–
In addition to insurance coverage as specified in Section 00300, both the Abatement Contractor and the
Demolition Contractor shall carry Contractor Pollution Liability Coverage with coverage of $1,000,000
occurrence, $2,000,000 aggregate, as a minimum. Abatement Contractor shall also carry asbestos
specific coverage under workers comp at $1,000,000 per occurrence, minimum.
Item No. 3–
Replace pages 3 and 4 in Section 00320, Proposal Form with the attached revised pages 3 and 4.
The Base Bid Nos. for the Lincoln Campus and the COM Senior Adult Center has been corrected to Base
Bid No 6 and Base Bid No. 7 respectively.

Add the following information on Hydro Mulch to Section 02310, Site Grading under paragraph 3.4,
Erosion Protection, page 3 as follows”
“C. Should alternate proposals for Hydro mulch be accepted, each site accepted shall be hydro
seeded with appropriate seasonal seed mix and maintained (watered) until substantial
growth (75%) has been established. Hydro mulch shall not occur until after final site
acceptance by Owner has been obtained. Hydro mulch shall be installed by a firm
specializing in such and shall be applied per manufacturer’s instructions and the site
stabilized in a sufficient manner to close out the Storm Water Pollution Prevention Plan
(SWPPP).”
