What to bring on the first day
Every child needs:
1. Completed enrollment paper work










Child enrollment application
Enrollment contract
Activity form
Allergy/Food permission form
Stroller/Walker form
Influenza form – just the part where you sign
Getting to know your child
Permission to photograph form
Topical ointment authorization form
Infant care plan – if applicable
2. Immunization and Physical form from Physician
3. Annual registration fee of $105.00
4. One complete change of clothes (shoes, socks, shorts, t-shirt, and
underwear if applicable) in zip lock bag.
 Infants need to have 3-4 outfits on hand.
 If your child is newly potty trained, we ask that 3-4 pairs of shorts,
underwear, socks and an additional pair of shoes be sent in as well.
5. 1 Small blanket, and 1 fitted crib sheet for nap time
6. 1 box of tissues
7. 1 package of face wipes
8. 1 container of play dough in your child’s favorite color
9. INFANTS ONLY – clean, empty bottles
Every item brought to school MUST be labeled with your
child’s FIRST AND LAST name.
