DAUGHTERS OF THE AMERICAN REVOLUTION
SCHOLARSHIP APPLICATION
The Daughters of the American Revolution (DAR) is an organization
dedicated to patriotism, historical education, and historical preservation,
with a strong commitment to community and service to others. It lives by
the motto: “Preserve the Past, Enhance the Present, and Invest in the
Future.” In that spirit, the Oregon Dunes Chapter of DAR is offering the
following scholarship.
QUALIFICATIONS:
This scholarship of $500 is available to a member of the current graduating
class of Siuslaw High School. The DAR is looking for students who have
turned their educational life around, or have overcome an obstacle in their
high school career. Consideration will be given to students who demonstrate
successful grades in their last two years of high school, can show
involvement in their community and family, and can demonstrate clear
educational and personal goals for their future.

ACADEMIC RECORD: Please attach a copy of high school transcript. Discuss
your high school career. Include any setbacks and how they were overcome.
What caused your attitude change or set you on your present course?
DISCUSS YOUR EXTRACURRICULAR ACTIVITIES: Include hobbies and outside
interests.
WORK EXPERIENCE: Where? When? For how long?
USE AS MUCH SPACE AS NECESSARY TO ANSWER THE FOLLOWING
QUESTIONS:
1. Describe your goals for the future. Why did you choose this institution?
How do you plan to achieve your goals?
2. Explain your monetary needs. What are you resources and how will you
contribute to them?
3. Discuss your involvement in your family, school, and community and how
these experiences will help you succeed in the future.
4. Where do you see yourself in 5 years?
LETTERS OF RECOMMENDATION: Include two letters of recommendation.
One should be from a teacher and the other may be from a person in the
community who knows you well. Each letter should include comments
about student’s responsibility, integrity, personality, and ability to pursue
goals and overcome obstacles.

USED: Scholarship will be mailed directly to the institution after the
successful completion of one term/semester based on a transcript of grades.
