My son/daughter, _________________________________ has my permission to try out for the West
High Cheerleading Squad. I understand that he/she must abide by the rules and regulations set forth by
the coach, athletic director, and principal of West High School, and be present for all practices and
scheduled activities. I have read the information provided and understand that the violation of any of
the rules and guidelines may lead to disqualification from tryouts. I understand that all required forms
must be completed and submitted by the first clinic, or my child will not be allowed to participate. I
understand that my child must attend at least one clinic and the formal tryouts, or my child will not be
considered for a cheerleading position unless prior arrangements have been made.
I understand that my daughter/son will be evaluated by qualified judges, but that the coach will have
the final word on squad placements, and we agree to abide by the decision of the coach.
I understand all costs involved as stated in the rules.
I understand that by the very nature of the activity, cheerleading carries a risk of physical injury. No
matter how careful the participant and coach are, how many spotters are used, or what landing
surface is used, the risk cannot be eliminated. I understand these risks and will not hold West High
School or any of its personnel responsible in the case of accident or injury at any time.
