ACADEMIC PROBATION SANCTION SHEET

EPSILON PHI
First Term:










She is ineligible for nomination or election of any chapter office or appointment as any chapter office or chair. If she is already
elected or appointed as an officer or chair, she may keep the position for the current term, providing:
A. She has a higher cumulative average than the minimum GPA (2.75 for members, 3.0 for Executive), and
B. The Executive Board votes its approval
She must attend an Individual Scholarship Meeting with the Scholarship Chair and Advisor.
This will be followed by a Standards Council Meeting with the Standards Council in which the member will commit to the
individualized program by signing a probation contract.
She will no longer have voice or vote at Alpha Delta Pi chapter meetings.
She must submit a Weekly Status Report each week to the Scholarship Chair.
She must be a part of the Risk Management Team during Alpha Delta Pi sponsored mixers/formals.
She is prohibited to take a diamond sister if her Cumulative GPA is below 2.75 OR her semester GPA is under 2.0
She is prohibited to “going out” on Wednesday Nights
She must submit mid-term grades to Scholarship Chair within one week of mid-term grades being reported.
She must have an informal meeting with the President, Standards, and Scholarship Chair at the point of midterms, and her
midterm grades be reviewed. If she has the minimum GPA of 2.75 at midterms, then her academic probation will be
terminated and her first offense will be recorded. If she fails to meet the minimum GPA of 2.75 at midterms, then the
above sanctions will stand for the remainder of the semester.
Second Term:














She must attend an Individual Scholarship Meeting with the Scholarship Chair and Advisor.
This will be followed by a Standards Council Meeting with the Standards Council in which the member will commit to the

She will no longer have voice or vote at Alpha Delta Pi chapter meetings.
She must relinquish any chapter office held.
She must be a part of the Risk Management Team during Alpha Delta Pi sponsored mixers/formals.
She must submit a Weekly Status Repot each week to the Scholarship chair.

She is prohibited from taking a diamond sister.
She is prohibited from participating in more than one intramural sport for the entire semester.
She is prohibited to “going out” on Wednesday or Thursday Nights
She is prohibited from attending more than 2 bid day parties
She may only take on a minimal role (non requirement to be at every practice for Lip Sync and/or Greek Sing)
She must submit mid-term grades to Scholarship Chair within one week of mid-term grades being reported.
She must have an informal meeting with the President, Standards, and Scholarship Chair at the point of midterms, and her
midterm grades be reviewed. If she has the minimum GPA of 2.75 at midterms, then her academic probation will be
terminated and her second offense will be recorded. If she fails to meet the minimum GPA of 2.75 at midterms, then the
above sanctions will stand for the remainder of the semester.
Third Term:
If there is sufficient evidence that the member failed to comply with the conditions of her scholarship probation from a previous
consecutive term that has been reported by the Scholarship Chair or Advisor; the Executive Board may vote to recommend cancellation
of membership as a result of a Formal Hearing.















She must attend an Individual Scholarship Meeting with the Scholarship Chair and Advisor.
This will be followed by a Standards Council Meeting with the Standards Council in which the member will commit to the
individualized program by signing a probation contract.
She will no longer have voice or vote at Alpha Delta Pi chapter meetings.
Participation in chapter events is restricted to philanthropic and sisterhood activities only. Participation in chapter social,
intramural and other events (e.g., Greek Week, Homecoming, Lip Sync etc.) will not be allowed.
Participation in chapter recruitment events will be reviewed with the Providence Director for permission(s).
She must relinquish any chapter office held.

terminated and her third offense will be recorded. If she fails to meet the minimum GPA of 2.75 at the time of final
grades, then she will be up for membership cancellation vote by the Executive Board.
Additional Assistance:
A. Required hours of study are the following:
o Semester GPA 2.74-2.5
+ (2 hours)
o Semester GPA 2.49-2.25
+ (4 hours
o Semester GPA 2.24-2.00
+ (6 hours
o Semester GPA 1.99-1.59
+ (8 hours
o Semester GPA 1.58-below
+ (10 hours
B. 1.99 and below will be prohibited from participation in Lip Sync and/or Greek Sing.
C. She shall seek assistance at one of the following resources within the first 6 weeks. Proof of attendance
should be reported to the Scholarship Chair. Identify assistance below:
a. Attend a scholastic focused workshop.
b. Participate in a counseling session at a campus academic resource center.
c. Arrange Tutoring for at least one class.
D. She must have documented attendance with the professor’s signature for each class, every day of every
week.
E. She must meet with a professor, teaching assistant or advisor twice during the term.
F. Additional probationary terns as determined during the Scholarship meeting:
