Clarence City Council joined the World Health Organisation (WHO), Global Network of Age friendly Cities and Communities in 2014, as the first and currently only Tasmanian city to do so. The network currently includes: 210 cities and communities in 26 countries, covering over 88 million people worldwide. www.ccc.tas.gov.au/positiveageing 9 Age Friendly is... A place that enables people of all ages to actively participate in community activities A place that treats everyone with respect, regardless of their age A place that makes it easy to stay connected to those around you and those you love A place that helps even the oldest people stay healthy and active A place that helps those who can no longer look after themselves to live with dignity and enjoyment A place for all ages – children, youth, older people A place for all – all ages, all abilities, and all backgrounds Council supports this. Instead of viewing ‘age’ as ‘old’, we place an importance on an age-friendly spectrum in our planning and services, such as Asset and Recreation, Children’s Services, Youth Services, Health and Wellbeing, Cultural Arts, History and Events, Access, and Positive Ageing. 10 Contact Information and Resources Clarence City Council – Positive Ageing www.ccc.tas.gov.au/positiveageing World Health Organisation (WHO) Age Friendly www.who.int/ageing/projects/age_friendly_cities_network WHO Age Friendly World www.agefriendlyworld.org COTA Tasmania Age Friendly Communities (Facebook page) www.facebook.com/pages/Tasmanian-Age-FriendlyCommunities Australian Network of Age-friendly Cities and Communities www.facebook.com/groups/anafcc 11 Council has a number of wonderful products, programs and facilities to help support its growing older population across the City. Now, if we have access to a telephone and a computer we have information available ‘at our fingertips’. My Aged Care was introduced on 1 July 2013 to assist older people and their families and carers to access aged care information and services. The My Aged Care website was established by the Australian Government to help navigate the aged care system. My Aged Care is part of the Australian Government’s changes to the aged care system which have been designed to give people more choice, more control and easier access to a full range of aged care services. My Aged Care is made up of a website and a contact centre. Together they can provide you with information on aged care for yourself, a family member, friend or someone you’re caring for. You can call the My Aged Care contact centre on 1800 200 422 between 8am and 8pm on weekdays and between 10am and 2pm on Saturdays. The My Aged Care contact centre is closed on Sundays and national public holidays. Our needs change as we get older and sometimes we require some additional support to remain living at home. What is the Age Care Assessment Team (ACAT)? See page 22 What is a Home Care Package? See page 23 What is consumer directed care? See page 24 21 What is the Age Care Assessment Team (ACAT)? If you need some help at home or are considering moving into an aged care home, you may first need a free assessment by an Aged Care Assessment Team (ACAT). A member of an ACAT will talk to you about your current situation and work out if you are eligible to receive government-subsidised aged care services. You will need an ACAT assessment and approval if you have complex aged care needs and want to: ✦ access aged care services through any level of Home Care Package ✦ receive services through transition care ✦receive respite care in an aged care home (sometimes known as nursing home) ✦ move into an aged care home You will need a Home Support Assessment if you have entry-level aged care needs and want to access services through the Commonwealth Home Support Program, such as meals or transport. You won’t need a Home Support Assessment or an ACAT assessment for aged care services that are not subsidised by the Australian Government such as services offered by volunteer groups and charitable organisations. Aged Care Assessment Team (ACAT) Level 1, 90 Davey Street, Hobart Phone 6222 7274 [email protected] 22 What is a Home Care Package? The Home Care Packages Program is part of the Australian Government’s continuum of care for older Australians. It provide older people who want to stay at home with access to a range of ongoing personal services, support services and clinical care that help them with their day-to-day activities. The Program sits in between the Commonwealth Home Support Program (formerly known as the Home and Community Care (HACC) Program) and Residential Aged Care. What levels of care are available? There are four levels of Home Care Package: ✦ Level 1 – to support people with basic care needs ✦ Level 2 – to support people with low level care needs ✦ Level 3 – to support people with intermediate care needs ✦ Level 4 – to support people with high care needs What types of services are provided? The services that can be provided in a Home Care Package include, but are not limited to: Support services – such as help with washing and ironing, house cleaning, gardening, basic home maintenance, home modifications related to your care needs, and transport to help you with shopping, visit your doctor or attend social activities Personal care – such as help with showering or bathing, dressing and mobility Nursing, allied health and other clinical services – hearing services and vision services Care coordination and case management – helping to make sure you receive holistic services tailored to your needs 23 What is consumer directed care? Since 1 July 2015, all Home Care Packages are being delivered on a Consumer Directed Care (CDC) basis. If you are already receiving a Home Care Package, your service provider will discuss what this means with you at the next review of your care plan. CDC means: ✦ you get more say in the care and package you access, how it is delivered and who delivers it to you ✦ you will have conversations about your needs and goals ✦ you will work in partnership with your service provider to develop your care plan ✦ you agree to the level of involvement you will have in managing your care package ✦ you will have a greater understanding about how your package is funded and how those funds are spent through your individualised budget and monthly income and expense statement ✦ your service provider will monitor and provide you with ongoing formal reviews to ensure that your package still meets your needs 24 Home Care, Residential Care, Respite Care and Lifestyle Villages The following organisations and services can help you make choices about how to access home care packages, or to select residential, respite or village lifestyle options. Through age, illness or accident, people may lose their capacity to understand the nature and consequences of proposed medical treatment. Healthcare decisions then fall to families, friends and health professionals who may not be aware of your wishes. You can create an advance care plan that may include: ✦ an Advance (Health or Care) Directive (or other similar document) ✦ an enduring power of attorney (or other similar document) ✦ a letter to the person who will be responsible for your decision-making ✦ an entry in your medical record ✦ instructions which clearly voice your view The advance care planning process encourages you to reflect on what is important to you, your beliefs, values, goals and preferences in life. It explains how you want to be cared for if you reach a point where you cannot communicate decisions about your medical care. It keeps you involved in your medical decisions, both now and in the future, whether you are healthy or have an illness. Each state and territory has different names for the legal documents you can use to create an advance care plan. 35 For more information: Hospice at Home (h@H) The District Nurses provide 24 hour support coordinated through a single call centre for advice and ehealth initiatives to enable assessment and deployment of local on-call nursing staff. 2 Birdwood Avenue, Moonah Phone 1800 4677 423 [email protected] Website www.hospiceathome.org.au Tasmanian Association for Hospice and Palliative Care (TAHPC) TAHPC is the peak representative body for hospice and palliative care in Tasmania and is affiliated with Palliative Care Australia.
