STUDENT TALENT SHOW 2014: AUDITION / PERFORMANCE CONTRACT
Please read all rules and regulations. Student and parent must sign, or you can’t participate.
This form must be turned in with your Student Info Sheet by Friday, 3/28 at 3:30 p.m.






Performers must be enrolled students at Sam Houston High School to participate.
Performances must be under 4 minutes.
Performers must be passing all classes to try-out and perform. A grade sheet must be completed
and turned in at try-outs April 1st and dress rehearsal April 15th . Failing grades or failure to submit
grade sheets will cause the performer to be removed from the Talent Show.
Performers must have good citizenship. From the day of auditions until showtime, any performer
that is assigned to OCS, Choices, Turning Point, lunch d-hall, or has any other citizenship issue will be
removed from the Talent Show.
Attendance – The student must have no more than three unexcused absences in any class.
Attendance will be checked before auditions, and will be checked again before showtime.
By signing the contract, you acknowledge that any bodily damage/injury sustained during try-outs,
rehearsal, or the Talent Show is not the responsibility of Sam Houston High School. Also, SHHS is not
held responsible for any broken or stolen property of the performer.
Auditions : April 1st & 2nd, Auditorium, 3:00-5:00 p.m.
 You must bring the following items to the audition:
o Grade/Citizenship Check form
o Lyrics of any song used in your performance.
 Do not be late. If your tardiness impedes the progress of auditions, you and your group will be
dropped from the show.
 All acts must be fully prepared for the audition—words memorized, choreography completed, etc.
Arrive early to try-outs. When you try-out, please move to the stage quickly. Set up quickly. Tell
the judges your try-out # and begin. When your performance is complete, please tell the judges
“Thank You.” This will tell the judges you are finished performing.
 We will video the auditions and rehearsals. By signing this form, you acknowledge the rules and
authorize the release of the photos and videos taken for the show. There is a possibility that there
will be a try-out/rehearsal video shown during the program. If you don’t make the show right away,
you still might make it if another group fails, etc.
 Costumes at the try-out must follow school dress code.
 ALL LYRICS MUST BE APPROPRIATE IN EVERY WAY. This includes any song being sung, performed by
an instrumentalist, or danced to. A “clean” version of a song with explicit lyrics is not acceptable (no
bleeped out words)—the original song must be appropriate in every way! All dances must be
appropriate, as well. This is a family show.
 Any acts using recorded music must bring two CDs to try-outs. Make sure that the CDs can be
played on any CD player! (Note: If you edit your music in GarageBand you must choose the “Burn
Song to CD” option from the Share pull-down menu to create an audio CD.)
 CDs must be labeled with all of the group members’ names and the name of the act.
Dress Rehearsal: April 9 – Auditorium – 3:00-5pm
 You must bring completed grade/citizenship sheet.
 If you don’t attend dress rehearsal, you or your group will be dropped from the show
 Do not be late to Dress Rehearsal. If your tardiness impedes the progress of dress rehearsal, you
and your group will be dropped from the show.
 What you wear at the dress rehearsal will be what you wear at the show. School dress code will still
be followed.
 Dress Rehearsal will be video- taped.
Student Talent Show – April 11 – Auditorium - 7-9pm – Tickets $3 at school and $5 at door
Statement of Responsibility
 If you are chosen to be in the show, you will have to behave appropriately on stage. We understand
that some elements of hosting or performing in the Talent Show may be unplanned or spontaneous
due to the crowd, but as a performer you are expected to be appropriate at all times. If you use foul
language or make inappropriate gestures, your performance will be stopped and you will be
referred to your Assistant Principal for disciplinary action.
 Your signature indicates that you agree to stay true to your commitment as a performer. You agree
that, you will act appropriately on stage during the show, and will perform exactly what you
performed in the auditions. You also agree to submit a detailed description of your entire
performance including all music titles, lyrics, scripts, act descriptions and costume descriptions. You
will not deviate from this description, unless given permission by Ms. Hader or Mr. Ostrowski. Your
signature indicated that you agree to all rules and regulations of the 2014 Student Talent Show.
