With the care of our patients becoming increasingly complex, we regularly reach bed capacity. Multiple patient transfers occur between units in order to ensure care is met with the right nursing skill set. Patient diversions to other facilities continue to occur. The purpose of this guideline and policy is to have a strategic focus on patient placement anywhere in the inpatient setting to improve quality care, reduce transfers and multiple handoffs, and expand nursing competency on Med/Surg units to accommodate needs. In this new process, the admitting physician and/or the administrative coordinator will collaborate to determine best placement for the patient. Guidelines define preferred unit placement based on specific patient diagnosis and/or related conditions and the unit’s nursing competency. Intermediate Care (IMC) status and non-invasive ventilation care has been expanded so these patients can be cared for throughout all Med/Surg units. Education is in progress to include telemetry monitoring skills for the 6 Med/Surg unit nurses, to better care for patients with ingestions and anorexia. If it is unclear where to place a patient based on diagnosis, complex condition or available beds per policy, the administrative coordinator will gather an interdisciplinary team of the admitting physician, unit medical and nursing directors, unit manager and unit charge nurse to confer on where the best placement for the patient would be. The administrative coordinator will record decisions made by this group. PULSE ARCHIVE | CLICK HERE FOR PAST ISSUES OF THE PULSE.
