POST DISCHARGE OUTCOME MEASURE PROGRAM – Discharges 2014
Contacts in 2016 (24 months after discharge)
We had a total of 298 discharges in dorms, ILP and group homes during the year 2014. We had a total of 174 clients staying more
than 3 months in our program. This is the benchmark for contacting them post discharge. Meaning, if a client attended our program
for more than 3 months, we try to contact him/ her over a period of two years after discharge no matter what the discharge reason
was. We were able to reach 53 out of the 174 former residents we were trying to contact. Sometimes we were only able to contact a
family member; therefore, many questions are unanswered. Nevertheless, the contact success rate is 30%.
Answers are not mandatory so total of answers vary from one question to another. Also to consider, family members are not able to
answer all questions.

SUMMARY OF POST DISCHARGE ANSWERS at the 24- month mark, fifty-three calls were made.
 Fifty- three former residents or family members were reached at the 24-month mark.
 Thirty- six former residents were LA County, seventeen were out of County
 At the 24-month mark, seven were in jail, four were in placement, two were out of state, two were homeless
according to family, and one was in camp.
 Nineteen former residents were attending school.
 Eleven former residents were working.
 Twenty- four of them where not on drugs or alcohol.
 Only five former residents were involved in gangs at the 24- month mark.
 One former resident lives in Alabama and has two kids, another one lives in Pennsylvania and is doing well
according to mother.
 Two former residents were shot and one got stabbed and have recovered.
 Two former residents had severe mental health issues: one was diagnosed with schizophrenia and the other
one was at high risk of suicide.
 One former resident was incarcerated and charged with murder.
