Dear Eligible Participant:
The NOITU Insurance Trust Fund was recently informed by BlueCross BlueShield that both CarePoint Health
hospitals - the Bayonne Medical Center and Hoboken University Medical Center - have terminated their contracts
with BlueCross BlueShield. As of January 1, 2016 the Bayonne Medical Center’s hospital contract was terminated.
And the Hoboken University Medical Center’s hospital contract terminated as of February 2, 2016.
Based on New Jersey law which governs the contracts for these two hospitals, members may use the facilities for
up to four months following the contract termination. This means that members can still access the Bayonne
Medical Center through April 30, 2016 and the Hoboken University Medical Center through June 1, 2016. Prior
authorization must still be obtained when required.

How do these changes affect my benefits?
After the dates listed above, these two hospitals would be treated as any other out of network facility and would be
reimbursed as such. This means, assuming you have out of network coverage; you may be subject to the Out of
Network deductibles and coinsurance up to your out of pocket maximum for any services rendered in these
hospitals. Most important – when a hospital is no longer a “participating facility” they can charge whatever they
wish. Please understand that the Insurance Trust Fund can only consider “Allowed, Eligible Charges” when paying
for out-of-network services (refer to your” Summary Plan Description” for more detail). This means that once the
Fund pays the maximum it can pay the Hospital can bill you for the difference between what the total charge is and
what the Fund’s maximum allowance is.
What should I do in a medical emergency?
In the case of medical emergency, common sense dictates that you should go to the nearest emergency facility
without delay.
What if I already have a scheduled procedure?
If you have a scheduled procedure and your expected discharge date is prior to April 30, 2016 the Bayonne Medical
Center or prior to June 1, 2016 at the Hoboken University Medical Center, there is no need to reschedule. However,
if your expected discharge date is after these dates, you should consult with your physician to consider scheduling
the procedure at a different BlueCross BlueShield contracted facility to avoid potential out of network costs that
you will be responsible to pay.
Is there a chance these facilities may rejoin the BlueCross BlueShield network?
BlueCross BlueShield has not provided information why these two facilities terminated their contracts; however,
very often it is related to contract negotiations. If this is the case with these two facilities and a new contract is
signed, then there is a possibility of them rejoining the BlueCross BlueShield network. If this occurs then you will be
notified as soon as possible.

How do these changes affect my benefits?
Después de las fechas indicadas anteriormente, estos hospitales serán tratados como cualquier otro servicio fuera
de red y serán pagados como tales. Esto significa que, suponiendo que tenga cobertura fuera de la red; usted puede
estar sujeto a deducibles de fuera de red y co-seguro hasta el máximo de su bolsillo por los servicios prestados en

reprogramar. Sin embargo, si su fecha de alta esperada es después de estas fechas, usted debe consultar con el
médico para que considere la posibilidad de programar el procedimiento en otras instalaciones que estén
contratadas con BlueCross BlueShield" para evitar posibles gastos de fuera de la red que usted será responsable de
