Bone Health
Bone tissue is some of the most dynamic in the body, constantly changing due to the pressures
of the surrounding environment. In healthy bone, matrix remodeling is a continual process
affecting 10 percent of bone mass at any given point. Balanced bone remodeling requires
robust nutrient availability to build bones with mineral density – a protective factor as we age.
Bone mineral density (BMD) acts as an important clinical tool for both assessing bone health
and evaluating disease risk; it refers to the mineral content per square centimeter of bones.
Evaluating for BMD is only indicated for those exhibiting risk factors for weak bones: women
aged 65 or greater, men aged 70 or greater, or those over 50 years with either previous
bone fracture, rheumatoid arthritis, low body weight, or a parent with a hip fracture. In
addition, individuals who have received long-term steroid therapy (prednisone) or those
with a history of vertebral abnormalities, primary hyperparathyroidism, disordered eating,
or osteoporosis drug use should receive monitoring. Other considerations such as smoking
and drinking habits or dietary deficiencies are addressed below.

Addressing Modifiable Risk Factors
Vitamin D deficiency is associated with mild increases in parathyroid hormone leading to increased bone resorption and lost BMD. There is
a positive association between 1,25-dihydroxycholecalciferol (the bio-active form of vitamin D) and BMD. When checking labs, be sure to
include both 1,25-dihydroxycholecalciferol (active form) and 25-hydroxycholecalciferol (reserve pool) to understand hormone status. Liver
congestion and chronic kidney disease are common causes for deficient conversion to the active form. The optimal choice for improving
vitamin D deficiency, High Potency Omega Plus (SOP) provides vitamins D3 & K2 to stimulate osteocalcin to lay down coupled calcium and
vitamin D in the bone matrix, building stronger bones in the process. Pure Vitamin D3 is also available in Vitamin D3 1000 (VD3) and Vitamin
D3 5000 (VD5000) to accommodate a full spectrum of dosing preference.
Alcohol consumption may improve bone mineral density when kept to a maximum of two drinks per day. The benefits include reduction of
cardiovascular events as well. Tobacco smoking is a primary risk factor for osteoporosis. While the mechanisms remain somewhat unclear,
smoking may inhibit osteoblast activity, which would decrease the amount of new bone produced. Additionally, smoking increases the
breakdown of estrogen, lowers body weight, and heralds menopause, all of which contribute to bone loss. The cadmium in cigarettes may
also encourage BMD loss. Accordingly, consider smoking cessation treatment. Anti-Smoking I (C7) supports treatment while patients attempt
to quit smoking, and Anti-Smoking II (C8) may be used once smoking has completely stopped.
Nutrition plays a key and complex role in bone mineral density. Ideal intake of dietary calcium, phosphorus, magnesium, zinc, boron, iron,
fluoride, copper, and vitamins A, D, E, K & C help to maintain healthy bone. Excess sodium serves as an independent risk factor. Low protein
intake is associated with lower peak bone mass in adolescence translating to lower bone mass in the elderly population; paradoxically, high
protein diets by themselves tend to raise blood acidity antagonizing bone tissue.To support both nutrient and pH status, supplement with Alka

Osteopenia & Osteoporosis
Osteopenia is a condition of lowered BMD considered by many
to be a precursor to the development of osteoporosis. It is
defined as a T-score between -0.1 and -2.5. While osteopenia
occurs most frequently in postmenopausal women, it should
also be considered in young female athletes where it is associated with Young Female Athlete Triad Syndrome. The other
two components of the triad include disordered eating and
amenorrhea. When a menstruating woman develops amenorrhea and it persists for greater than six months, she loses
bone mineral density just as a postmenopausal woman does.
Many female athletes tend to have lower body weight as well
as lower body-fat; depleted energy and a low-fat diet can suppress her estrogen levels prompting the BMD loss. Additionally,
it is not uncommon for this estrogen deficient state to prompt
menopausal type symptoms including hot flashes and night
sweats. Consider a diet high in healthy fats to ensure proper
cholesterol status for the production of steroid hormones
including estrogen. Many women see improvement in their
BMD status by maintaining a gluten-free diet.
High Potency Omega Plus SOP

enhance the absorption of fat-soluble vitamins D3 & K2 for improving
BMD and reducing calcium plaque build-up.
Osteoporosis comes from Greek translating as porous bone;
and it is defined by a T-score of less than -2.5. The condition
itself has no symptoms; the main concern is increased risk of
fracture from the bone mineral density loss and architectural
changes to the bone matrix. Of the three types of osteoporosis,
primary type 1, or postmenopausal osteoporosis, is the most
common. Primary type 2 affects women and men over the age
of 75 at a comparable incidence rate of 2:1 and is sometimes
referred to as senile osteoporosis. Secondary osteoporosis
can occur at any age and affects women and men equally.
While this last type may be the result of underlying disease, it
may also develop after the use of glucocorticoids or steroids

reduction of disease include diet, exercise, and minimizing or
preventing falls. Bone fractures are the hallmark of this disease
and commonly affect the vertebral column, hip, rib, and wrist.

Fractures associated with osteoporosis can be asymptomatic
or cause debilitating acute or chronic pain as well as a stooped

decreased mobility. Fractures of the long bones not only contribute to decreased mobility but may also require prompt

Osteodystrophia Deformans
Osteodystrophia Deformans is perhaps better known as Paget’s
disease of the bone. Affected bones undergo excessive breakdown causing them to weaken and misshapen. A person may
be asymptomatic, but the condition is frequently accompanied by
pain and pathologic fractures with arthritis in the nearby joints.
Oftentimes, Paget’s is only discovered after complications have
developed. Paget’s disease is a localized pathology affecting one or
just a few bones as opposed to osteopenia or osteoporosis which
affect the global health of bone tissue. Rarely diagnosed before
the age of 40, Paget’s disease of the bone occurs more frequently in
men than women (3:2). It is considered very difficult to treat from
a conventional standpoint and Sir James Paget first suggested the
disease was due to an inflammatory process. Newer evidence
suggests he may be right; additionally, there may be underlying infection with a paramyxovirus. No one pathogen has been identified yet,
but the three most common pathogenic agents are measles, canine
distemper virus and respiratory syncytial virus – each of which can
be found within Professional Formulas’ homeopathic product line.

Osteomalacia & Rickets
Osteomalacia and Rickets are similar disease processes but
osteomalacia is specific to adults while rickets is specific to
children. Characterized by softening of the bones due to defective mineralization, it may resemble osteoporosis on the
surface; however, the two diseases are significantly different.
The two main causes of osteomalacia are insufficient calcium
absorption (due to deficient dietary intake or a deficiency or
resistance to vitamin D activity) or a phosphate deficiency
caused by increased renal loss. The major finding of this disease
is low vitamin D status but other key findings are low serum
and urinary calcium, low serum phosphate, high serum alkaline
phosphatase and elevated parathyroid hormone. Elevated alkaline phosphatase is directly associated to the rate of bone
turnover.
In adults the disease progresses insidiously, starting as aches and
pains in the lumbar region and thighs and later spreading to the
arms and ribs. The pain is classically symmetrical, non-radiating
and accompanied by a feeling of sensitivity in the involved bones;
it is not spontaneous but revealed through applying pressure.
The proximal muscles are weak with a difficulty to pull oneself
up during activities like sitting up or climbing stairs. A waddlinglike gait develops perhaps accompanied by deformities such
as triradiate pelvis or lordosis of the spine. It is important to
note that once bones deform, they do not regain their original
shape. Pathologic fractures may develop, but most of the time
the predominant symptom is chronic fatigue.
Nutritional osteomalacia is conventionally treated with 10,000
IU of vitamin D weekly for four to six weeks. Consider daily
dosing in malabsorptive individuals or as an effort to keep the
serum saturation as even as possible.
Rickets is among the most common childhood diseases in
many developing countries and, for different reasons (specifically sun avoidance behavior and sunscreen use), it is rising
again in developed countries -. In either case vitamin D deficiency is the primary cause. Foods high in vitamin D include
butter, eggs, fish liver oils, portabella and shitake mushrooms, and
oily fishes such as tuna, herring, and salmon. UVB light exposure
on unprotected skin is necessary for endogenous production
of vitamin D, though precaution should be taken to avoid burns;
darker skin will need longer exposure to UV rays for similar
effect. Recommended supplementation for infants and children
is 400 IU of vitamin D daily and must be combined with dietary
calcium and phosphorus for best results.

Primary Hyperparathyroidism
Primary Hyperparathyroidism causes hypercalcemia through
the excessive secretion of parathyroid hormone and is usually
caused by a sporadic, single adenoma of the parathyroid gland
resulting from clonal mutation. The signs and symptoms are the
same as hypercalcemia and a favorite mnemonic of medical
students: stones, bones, abdominal groans and psychiatric
moans.
• Stones: renal stones, nephrocalcinosis and diabetes insipidus
(polyuria and polydipsia). Renal failure is a complication.
• Bones: bone related complications such as pain and pathological fractures, osteoporosis, osteomalacia and arthritis.
• Abdominal groans: constipation, indigestion, nausea and
vomiting, peptic ulcers and acute pancreatitis.
• Psychiatric moans: effects on the central nervous system such
as lethargy, fatigue, depression, memory loss, psychosis, ataxia,
delirium or coma.
• Others include: left ventricular hypertrophy and increased
all-cause mortality
Diagnosis is by blood test and will show elevated serum calcium.
Parathyroid hormone will also be high, where the expected
finding would be low in response to the elevated serum calcium.
Primar y hyperparathyroidism is a feature of several familial
endocrine disorders and in all cases is considered idiopathic.
Complications include the bone disease osteitis fibrosa cystica
which can result in pain and pathological fractures, osteoporosis, osteomalacia and arthritis. Surgical removal is very effective
treatment.

Osteomyelitis
Osteomyelitis is inflammation of the bone marrow specifically
related to an infectious process, and occasionally subcategorized
by causative organism (pyogenic bacteria or mycobacteria), route,
duration and anatomic location of the infection. Microorganisms
generally infect the bone through three main methods: the
bloodstream, local infection such as cellulitis or penetrating trauma
including joint replacement. After the bone is infected, leukocytes
enter the affected area to phagocytose the microorganisms and
in the process they release an enzyme that lyses the bone. The
infectious pus spreads into the blood vessels impairing flow rate
creating a chronic infection. The body attempts to wall off the
infection by laying down new bone around the necrotic tissue.
Chronic osteomyelitis may be due to intracellular organisms that
can easily evade the immune system and are frequently resistant
to antibiotics. Because of these factors, combining homeopathy
with the immune system support offered by Beta 1,3 D Glucan
may be an especially effective treatment approach. Additionally,
fungal infections can be a cause of osteomyelitis and for these cases
consider the combination of Beta 1,3 D Glucan with D-glucarate.

Renal Osteodystrophy
Renal Osteodystrophy is also known as chronic kidney diseasemineral and bone disorder (CKD-MBD). As a bone pathology,
it is characterized by bone mineral deficiency as a direct result
of the electrolyte and endocrine abnormalities that accompany
chronic kidney disease. Biochemically, it is thought to be the result
of hyperparathyroidism secondary to hyperphosphatemia and
hypocalcemia; both of these are due to decreased excretion
of phosphate by the damaged kidney. This situation is further
compromised by the damaged kidney’s inability to convert D3
to its active form, calcitriol, and results in further hypocalcemia.
It can be further defined as either low or high bone turnover.
The symptom profile is similar to other low BMD diseases
that may include joint pain or bone pain, deformity or fracture.
Diagnosis is usually after treatment for end-stage renal disease is
underway. Labs show low calcium and calcitriol with elevated
phosphate and parathyroid hormone; on x-ray alone it may be
difficult to differentiate from other low BMD conditions. Beyond
hemodialysis and transplant, treatment includes calcium and vitamin
D supplementation, restricted dietary phosphate (especially dairy)
and use of phosphate binders such as calcium carbonate. It should
be noted that recovery is observed in renal transplant patients.

Osteophytes & Enthesophytes
Osteophytes, also called bone spurs or even parrot’s beak,
develop along the margin of the bone as an ossified or bony
projection. They are classically associated to changes in bone
formation related to aging, degeneration, mechanical instability
or disease. Inflammatory damage to a joint surface, most commonly from arthritis initiates osteophyte growth and typically
leads to limited range of movement and pain. Osteophytes
commonly form along the spine as we age, and in this location
they are not likely to cause pain unless they grow to impinge
a nerve. Such cases may present as pain in the upper or lower
extremities or a sensation of numbness or tingling. When they
develop along the distal interphalangeal (DIP) joints of fingers
or toes, osteophytes are referred to as Heber den’s nodes
and as Bouchard’s nodes if along the proximal interphalangeal
(PIP) joints. Osteophytes may also be the product of deeper
disease like osteomyelitis or diabetes mellitus or more simply
the result of mechanical stress.
Enthesophytes are commonly mistaken as osteophytes, and while
they are another type of bony projection, they occur strictly at
the attachment of a tendon or ligament as a response to the
forces of mechanical stress.

Osteoarthritis
Osteoarthritis, while not a true bone disorder, is a degenerative
arthritis that can degrade the joint integrity enough to involve the
subchondral bone. It is the most common form of arthritis and
the leading cause of chronic disability in the US. Symptoms may
include joint pain, tenderness, stiffness, locking or effusion. The
pain is frequently described as a sharp ache or burning sensation.
