CHOICES Thomas Rd. clinic merges
with Arcadia and Enclave clinics
Effective Thursday, Dec. 2, 2010, the two teams and
service recipients from CHOICES Thomas Road Clinic
merged with CHOICES Arcadia and Enclave clinics.
Depending on where their team relocated, service
recipients from the Thomas Road clinic will be served
by either the Arcadia clinic or the Enclave clinic, unless
he or she opted to instead transfer to another clinic
location.
Magellan and CHOICES carefully studied how to best
restructure the system to maintain clinic accessibility
while retaining service quality given the recent funding
cuts approved by the Arizona Legislature for services
for non-Title 19 SMI individuals, who now will be visiting
the clinics much less frequently than before due to more
limited services being available to them.
The Arcadia clinic is located at 3311 N. 44th St., Ste.
100, Phoenix, AZ 85018, near the intersection of 44th St.
and Osborn Road and can be reached by calling (602)
957-2220. The Arcadia clinic is served by bus route(s)
44 (44th St.) and is within walking distance of bus
routes 41 (Indian School) and 29 (Thomas).
And, the Enclave clinic is located at 1642 S. Priest Dr.,
Ste. 101, Tempe, AZ 85251, near the intersection of
Priest Drive and Broadway Road, and can be reach by
calling (480) 929-5100. The Enclave clinic is serve by
bus route(s) 56 (Priest) and 45 (Broadway).

Join Magellan for a multicultural brown bag presentation where Barbara, Richard and Dick will tell
their powerful stories about living with a mental illness. This presentation will be held Wednesday,
