Security Incident Offshore Nigeria Security Incident offshore Nigeria • Incident Summary • In October 2012, while a Resupply vessel was on transit to the Survey area (having refuelled at sea away from the location) , she was boarded by 7 armed men at 38 Nautical miles south of Brass waters Bayelsa State. • The crew consisting of 7 expatriates and 9 Nigerians who secured themselves in the Citadel room of the vessel. • The armed men on boarding the vessel broke the bridge windscreen and gained entry into the vessel bridge and accommodation area and using a grinding machine broke into the Citadel room. • The armed men abducted all the expatriate officers on the vessel consisting of 6 Russians and 1 Estonian. • Valuables and food items were all stolen by the pirates. • Within 36-48 hours the Resupply vessel was escorted back to Port with 6 Nigerian Navy on-board and all the 9 Nigeria crew. • After an extended period of negotiation phase, the expatriates were released in the good health and repatriated. after sixteen days of detention. 2 Security Incident Location 3 Security Incident offshore Nigeria • Immediate Cause – • Failure to Identify Hazard/Risk • Contractor Master / Mgt –Didn’t recognize the risk of sailing closer to coast. • Root Causes – • Lack of Knowledge – Inadequate update training • Contractor Mgt - Inadequate approach to offshore passage. • Lack of Control – • Commitment, Leadership and Accountability • Contractor Mgt - complacency to threat of piracy • Action Items • Add expectation of agreeing risk level / controls to Security Plan 4 Security Incident offshore Nigeria • Immediate Cause – • Inadequate Guards or Barriers • Citadel was breached. • Root Causes – • Inadequate Engineering • Insufficient design of citadel. / • Lack of Control – • Risk Management • Prevention and mitigation measures. • Action Items • Establish Marine Offshore JM procedure • Add Security aspects to the FFP audit.. 5 Security Incident offshore Nigeria • Immediate Cause – • Failure to Secure/Protect • Lack of guard vessel with Supply Vessel during transit off project location • Root Causes – • Lack of Knowledge - Lack of situational awareness • Lack of assessment of passage risk for bunkering by Company in initial Project Security Plan. • Lack of Control – • Risk Management - Hazard analysis and risk control • Failure to identity risk associated with resupply run • Action Items • Update expectation in Security Plan template • Update expectation in Project Plan template • Contractor and Supplier Management • Only informal engagement with Contractor on risk tolerance level • Action Items • Add expectation to Security Plan 6 Security Incident offshore Nigeria • Immediate Cause – • Failure to Follow Policy/Standards/Procedures/Work Instructions/Practices/Rules Mother Vessel failed to instruct on more offshore route. (only general guidance given) • Root Causes – • Inadequate Leadership and/or Supervision- Giving inadequate procedure, practices, or guidelines • Failing to instruct the set requirement for southerly passage. • Lack of Control – • Contractor and Supplier Management • Company failure to exercise authority and responsibility • Action Items • Add expectation to Security Plan 7 Security Incident offshore Nigeria • Immediate Cause – • Failure to Check/Monitor/Observe • Company - No monitor of passage to the North west - instead of South from project • Root Causes – • Improper Motivation - Inadequate reinforcement of proper behaviour • Reluctance to intervene - having seen the incorrect route • Lack of Control – • Commitment, Leadership and Accountability • Mother vessel reluctance to intervene • Action Items • Add expectation to Security Plan • Establish Marine Offshore JM procedure 8 Security Incident offshore Nigeria • Immediate Cause – • Failure to React/Correct • No intervention when support vessel sailed North west and closer to shore • Root Causes – • Inadequate Leadership and/or Supervision - Unclear or conflicting assignment of responsibility • Company - Failure to recognize authority and responsibility to direct the Support vessel routing when off survey location. • Lack of Control – • Commitment, Leadership and Accountability • Company failure to give direction. • Action Items • Add expectation to Security Plan • Establish Marine Offshore JM procedure • Responsibility discussion at next field managers meeting • Share with IAGC 9 Contractor - Restricted Zone without escort (Supply Vessel) and passage plan Bonga Usan Passage plan without escort Tanker/cargo Supply Passenger/ fishing boat / JTF/ platform Agbami Akpo 10
