It should look like this: Amy Lee (Your name: first, then last) IEP Grammar— Swan (Subject, instructor’s last name) Conditionals, Homework (Specific Assignment Name) Apr 2nd, 2016 (Month, Date, Year) Course Requirements 1. Show respect to your teacher and your classmates. When someone is talking, look at them and listen. Do not talk while the teacher OR another student is talking. This is very serious. I will deduct participation points each time you show disrespect to your classmates. 2. Come to class on time every day. I expect you to be sitting in your desk ready to start class. If you are late, come into the classroom without being disruptive. Do not greet anyone. Find your seat and get to work! Note: coming to class late affects your participation score. 3. Take responsibility for your learning. You will pass this class if you show me that you have the English proficiency needed for the next level. You can help yourself by visiting the tutoring center, doing extra listening practice on your own, and keeping a vocabulary journal. 4. Do all of your own work. All assignments were designed to help you improve. Do not cheat yourself out of the opportunity to improve. Any copied or plagiarized assignment will receive zero points. 5. Turn in your assignments on time. 6. Come to class when tests are scheduled. There will be no make-up tests. 7. If you are absent, check the website and/or contact a classmate to find out what you missed. I will expect you to come back to class fully prepared for class. 8. Turn off your cell phone and/or other devices while you are in class. Cell phones tend to be distracting, which leads to showing disrespect to your teacher and classmates. 9. Do not try to negotiate with me for grades. If you believe I have made a mistake, please tell me. However, do not ask for a passing score that you have not earned. It would be unfair of me to pass a student because he or she has a scholarship or visa issues. 10. Do not bring food or drink into the classroom. The only exception allowed is a bottle of water. 11. Plan restroom breaks between class periods. Attendance Policy: *5 Absences = WARNING / 10 Absences = IT IS UNLIKELY THAT YOU WILL PASS THE CLASS NOTE: If you are absent: Email me and contact one of your classmates to see what you have missed. Missing class is not an excuse for not doing assignments. If you are going to be absent for 3 days or more, please email in advance to let me know. Class Contacts: Write the name and phone number/e-mail address of 2 classmates here. 1. Classmate: _____________________________ Phone Number: __________________________ 2. Classmate: _____________________________ Phone Number: __________________________ Academic Dishonesty & Plagiarism: Any type of academic dishonesty may result in a failing grade for the course. Examples: looking in the direction of another student's quiz/exam or computer screen, checking your cell phone during a test, copying, downloading and/or exchanging files, substituting another student's work for your own, using “cheat sheets,” etc. Please don't do it! ALWAYS DO YOUR OWN WORK! Note about disabilities and course accommodations (Americans With Disabilities Act): If you need course adaptations or accommodations because of a disability, if you have emergency medical information to share with me, or if you need special arrangements in case the building must be evacuated, please make an appointment to speak with me as soon as possible. If you need any special accommodation due to a documented disability or medical condition, I will help you arrange to meet with an International Student Advisor and the Disability Support Services Office so that accommodations can be arranged. Information about Emergency Preparedness Where can I find the SU Emergency Crisis Management Plan? The plan is online at http://www.seattleu.edu/safety/. What should I do if there is an emergency or I see something suspicious? Any threat or risk that puts lives in immediate danger should be reported to Campus Public Safety at 206-2965911. If you are off-campus, call 911. Lock-Down Steps: If the order to “Lock Down” is given, you should do the following: If you are in an office or a room that can be locked, close and lock the door, turn out lights, lay low and shelter behind any furniture against a solid wall. If the doors do not lock, stack furniture in front of the door, turn out the lights, lay low and shelter behind the remaining furniture against a solid wall. Avoid sheltering in an area in the room near or next to the windows. Wait for further instructions or messages that the threat has passed. If you are in an outside area, evacuate yourself away from campus to an open business or other safe area and take cover. Monitor for updates and the “all clear” signal from Public Safety or the police.
