Preparation of the acetabulum
Reaming
•
The true bottom of the acetbulum is exposed by initially reaming
vertically with a small reamer.
• Reaming at 45° is then undertaken using a reamer at least 2 mm
smaller than the diameter of the femoral head.
• The last size reamer is the same diameter as the implant chosen.
(The definitive implant is slightly larger around the equator to
ensure press fit).

Trials
A trial of the same size as the reamer allows to asses the fit and
position of the cup.
A positioning device allows vertical axis.
Polyethylen on metal bearings
Trial polyethylene liners are
assembled to the shell gauge.
Connect the snapfit handle to the trial liner ( a quarter turn bayonet
connection)
If a lipped liner is to be used, take note of the lip position.

Implanting the shell
OPTION : If the metal-back is installed without fixing screws, it’s
possible to add sealing plugs. On the table and with the metal-back in
the hand, use a hexagonal screwdriver.
Use the hexagonal joint screwdriver if a sealing plug needs to be
taken out of the metalback when in situ.
Affixing the shell to the shell impactor
The notch allows visualising the position of the implant so that the
indicator is placed in the superior part of the acetabulum.
Incline the cup at 45°.
Check the required anteversion before impaction.
Impact the shell.
It is not recommended to correct the orientation after impaction.
Ceramic on ceramic bearings
Trial within the definitive shell.
Connect the snapfit handle to the trial liner (a quarter turn bayonet
connection).
The trial liner can be trialled with either definitive or trial femoral
components in order to test for correct stability and neck length.
The assembly can then be reduced and tested.
The definitive head can then be chosen.

Preparation for acetabular screws
Choose a 3.2 mm drill bit. A drill guide is used to allow the drill to be
centered in the screw hole.
The pivot angle of the 6.5 mm screws is 15°.
Drilling is carried out with a 3.2 mm flexible drill shaft.
If the bone is sclerotic, a 4.5 mm drill may be selected.
A depth gauge is used to determine the length of the screws.
The range of 6.5 mm self tapping screws is 20 to 45 mm length.
The screws are inserted using a cardan-jointed screwdriver, screw
holding forceps may be used for additional stability when inserting
screws.
Polyethylen liner insertion
Impaction of the liner is accomplishes by one liner pusher head onto the
end of the acetabular shell handle.
After the appropriate liner has been impacted firmly against the
acetabular shell, only lipped liner is protruding over the shell.

Ceramic liner insertion
Check to ensure that no soft tissue or bone particles are present in
the inner diameter of the shell to allow for a maximum ceramic liner
interface.
The suction holder, in combination with the Quartz impactor, allows
secure handling of the ceramic insert, and its accurate positioning
within the Quartz shell.
Insert the ceramic liner only after ensuring the inner taper of the shell
is clean and dry.
The liner must be checked to make sure that it is oriented properly.
Impaction of the liner is accomplishes by one liner pusher head onto the
end of the acetabular shell handle. Only a gentle tap is required.
Repeated impaction may be avoided by assuring the liner is correctly
seated in, and flush with, the shell by running a finger around the
circumference of the liner/shell interface. The liner/shell interface
should be even without either component protruding over the other.
Ceramic liner extraction
Place the suction cup onto the inner surface of the liner and stretch
slightly above.
Place the prong on the removal tool successively in one of the three
dimples on the face of the shell.
Firm tapping on the removal tool handle will cause the taper connection
to loosen, and the suction cup will remove the liner.
Do not strike the liner !
Polyethylen liner extraction
Drill a 3.2 mm diameter hole in vertical axis.
Screw a 4.5 cortical screw through the liner.
