A Message from Our Board Chair and Chief Executive Officer
Greetings everyone:
As we reflect on 2014, we must remind ourselves that just 10 short
years ago, on October 1, 2004, and after several years of hard work,
15 employees opened the doors of Hospice of Southern Maine
(HSM) and began providing care to 27 patients and their families.

Since that time, our agency has grown to care for nearly 1,400
patients and their families annually throughout all of Cumberland
and York counties. The accomplishments highlighted throughout
this report reflect the passion and dedication of more than 140
employees, nearly as many volunteers, and several members from the
communities we serve.
I joined HSM in September 2014 at a very important time. The
agency was moving into its next decade of caring while growing into a mid-sized agency. But, even as we grow, our
mission remains clear: “Compassion, Care and Comfort Through End of Life”.
We believe that hospice care is not about giving up, it is about taking charge. We are committed to providing excellent
care through a team of highly trained compassionate experts, including hospice physicians, primary care physicians,
nurses, social workers, aides, chaplains, volunteers and bereavement coordinators, all of whom have patients and families
as their highest priority.
We are confident that HSM will continue to lead the way in providing end-of-life care in southern Maine. Thank you

who need our care.

Medicare and Insurance, net 92.5%
Hospice of Southern Maine served more than 1,400 patients in 2014, 5% more
than 2013. The financial support that allowed us to deliver end-of-life care to
patients and families was largely provided by health insurance. Medicare was our
major pay source. However, nearly 7% of our income this past year came from our
generous community in the form of unrestricted donations or grants. Community
support allowed us to deliver care to all. In 2014, Hospice of Southern Maine
provided $287,023 in uncompensated care. It also allowed Hospice of Southern
Maine to deliver care that was not otherwise covered, such as bereavement services.
Bereavement services were available to our Hospice of Southern Maine families as
well as any York and Cumberland county residents in need of grief support.

Our clinical staff managed to touch and care for more patients this year, while
maintaining their high quality of care. Our organization was challenged by an
unprecedented number of Medicare regulatory changes in 2014. The successive
changes were not easy to thread into our small administrative structure but we
were successful in creating new systems and processes to accommodate Medicare’s
requirements. These successes allowed us to maintain a positive bottom line.

WHAT WE DO
In-Home Program
Where we are needed…Hospice of Southern Maine’s
In-Home program provides services throughout all of
Cumberland and York counties as well as some outlying
communities. Our nurses, chaplains, social workers and
hospice aides travel all over the 1,826 square miles to
provide support and care to our patients and families.
Whether our patients are in nursing homes, assisted
living facilities, or their homes we help to make them
comfortable.

Gosnell Memorial Hospice House
We renovated! … Hospice of Southern Maine was very
fortunate to be the recipient of a grant to renovate Gosnell
Memorial Hospice House. The goal of the renovation was to
allow more space to meet with families, and provide more private
office space for staff. The new reception desk at the entrance
allows us to immediately greet and assist families when they enter
the building creating the warm welcoming experience Gosnell is
so well known for.

unfamiliar with hospice.”

Bereavement
Caring Comfort
Grief support…Each person’s grief is unique and
profoundly personal. Hospice of Southern Maine
recognizes how important the healing process is to those
that have lost a loved one. Our Bereavement program
provides individual and group counseling sessions to the
community regardless of whether or not their loved one
received our care.
Caring Comfort provides support for those that still
seek curative measures as they transition to qualifying
for hospice care. This program fills a need for those
that have a one year prognosis but don’t yet qualify for
hospice care. Our staff of volunteers assists with light
errands, rides and companionship.

A Volunteer’s story ~
“Helping people live
with dignity”
Cathy Bowden, was drawn to become a hospice volunteer
after experiencing her own personal loss. “I found that I had
a desire to help lift people up and support them through the
death experience.” said Cathy. “The gift is being present.”
As an In-Home program volunteer she has enjoyed helping
patients with the aspects of their life that aren’t part of their
medical and spiritual care. Volunteers like her, allow patients
to continue to be independent and do the things they love. “I
have learned how fiercely independent we all are right up to our death.”
As a Hospice of Southern Maine volunteer driver, Cathy helped a mother, who lived more than an hour away from
Gosnell Memorial Hospice House to visit her child who was in our care. “I could have said no, but something called to
me. I felt it was more important than my list of errands,” Cathy said, “I find helping people live with dignity is what’s
important to me.”

Hospice Experience
Our Story…
It all began when Joe started struggling with the aerobic
portion of his regular gym workout… At the advice of the
instructor, Joe saw his doctor that day.
Enter Hospice of Southern Maine…
Almost a year after the diagnosis, Joe collapsed. He
attempted to get out of a chair and could not do it. We
dialed 911 and Joe was transported to the emergency room.
When it was clear that Joe would continue to experience
debilitating infections, without hesitation he decided not
to be admitted and chose to go home with hospice care.
On September 29, he arrived home by ambulance. The
hospice admitting nurse arrived a few hours before him to
complete the admitting process prior to his arrival and talk
to me about what would happen next.

In the last couple of months we had a volunteer who
visited weekly. He also connected with Joe easily and was
able to relieve me to do errands.

We were fortunate to have two wonderful hospice nurses,
one our primary care nurse and a substitute when needed.
Both amazed us with their expert knowledge, sensitivity,
compassion, professionalism, problem solving and more.
The nurse visits were twice a week which included basic
assessments but more importantly listening, answering
questions, addressing concerns that had come up, and
telling us what to expect. Their experience in working with
clients with this diagnosis was invaluable. The ability of
both nurses to problem solve was impressive and of course
We also received a visit from our social worker who called
on several occasions at times that worked for us. She was
helpful and was able to answer all our questions including
what to do at the end.

I became fond of saying that with leukemia there is no
physical pain, no nausea, and no weight loss associated
with most cancers. However, there was no getting away
from it or ignoring what was looming and foreseeable as we
got closer to the end.
We also had a terrific hospice aide. She was thorough,
kind, efficient, a good match for Joe. She liked his sense
of humor and she once said she learned a lot when she
came to our home. I would hear them talk and laugh
and those experiences were very good for him. She never
hesitated to do the “extras” and always made sure his needs
were met before she left. She also recommended that we
request a lounge chair that hospice provides. (Joe just loved
reclining and watching news and favorite sports including
the Winter Olympics and the Men’s World Cup which he
feared he would miss.)

We had a brief window of time before starting treatment,
so we spent a memorable vacation at Niagara on the Lake
in Ontario with friends. For the next 15 months, Joe
received weekly palliative chemotherapy treatments. Since
there were no effects or adverse symptoms from the disease
or treatment, we lived fairly normal lives.

with whatever happened. For example, advising us to
continue medications that were absolutely necessary and
eliminating others. Joe thought that choosing hospice
meant no more medications, but we both learned that’s not
the case. Some medications, like those to prevent stroke or
soothe a rash, are beneficial and they even helped us get the
prescriptions.

Within days, we got the diagnosis: Acute Myeloblastic
Leukemia (AML), incurable, and treatable only with
palliative chemotherapy treatment. Joe asked the difficult
question anyone in his situation would want to know –
“How long do I have?” Her answer was six months to a
year with treatment. Telling our son Luke, and other family
and close friends this devastating news over the next few
days was enormously difficult. Although pale, Joe looked
and seemed quite healthy. This fed our inevitable denial.
Just a few weeks before, Joe had been active, involved, and
seemingly very healthy. He retired at age 77 from his full
time job as Executive Director of Shalom House, and at
age 81 from the Maine Senate. He loved being retired and
had so much to look forward to. fundraise for it. When Joe and I learned that Gosnell
provided respite care for home hospice patients, we were
absolutely thrilled. Joe finally got his wish to spend a week
at Gosnell for respite care.

On the first morning at Gosnell, the chef visited and asked
him what he wanted for breakfast and offered just about
anything anyone could want. When I called Joe later, he
told me he had the
“best breakfast” of
French toast and
bacon which he
thoroughly enjoyed! Joe ate quite
normally during his
illness, leukemia did
not affect his appetite
until the last few
weeks.
While at Gosnell,
Joe enjoyed visits
from friends and
family. The family
and patient focused
experience was so
refreshing. The staff and volunteers were available with
the touch of a button. The cookies and beverages were
available to clients and family alike. Joe particularly
enjoyed the frappes that were made to order.
On the day of the Super Bowl, I asked if it would be
possible to watch the game in the conference room, since
the TV screen was much bigger than in Joe’s room. Our
son, Luke, joined us, arranging the comfortable chair
for Joe and using his strength to transfer him from the
wheelchair expertly. The aide came a few times, brought
him a frappe and we quite enjoyed the game, especially the
outcome!

Joe’s Death…
Joe died at home on January 17, 2015, one year and
five months following his diagnosis. When he died, we
were again fortunate to have a nurse who exceeded our
expectations. She was new to us, yet her compassion and
demeanor could not have been better. She stayed a long
time, and was very respectful and sensitive to me, my son,
and our family and friends. She was also very considerate
in the post-mortem care she provided to Joe.
Along with my son Luke, I want to express our sincere
thanks and appreciation to all who are involved with
Hospice of Southern Maine. The expertise of the staff,
their care and compassion made a real impact on us. In
particular, Hospice of Southern Maine gave Joe the ability
to die at home the way he wanted to die. -Claire Brannigan has led a professional career in nursing, nursing education
and administration. She holds a MS in Nursing from Boston University. In
2001 and 2008 she taught for a RN to BSN Nursing program in Tel Aviv,
Israel. She has shared her expertise on numerous boards and committees in
nursing education and healthcare. She and Joe were married for 42 years and
have a son, Luke Brannigan.
At Gosnell, like at home, the nurses were informative,
helpful and available at all times. The admission nurse
completed the admission process with me before Joe
arrived which made it easier for him. Again, we were
impressed with the care and expertise provided by all the
nurses.
Joe’s Advocacy for Hospice…
Joe’s interest in hospice care started when he learned several
years ago that Maine was ranked 49 out of 50 for utilizing
hospice care. Joe decided to work to change that.
It became his passion to do something about this and he
did everything he could to effect change in hospice care in
Maine. Not only working to increase use of hospice care in
Maine but also to build an inpatient hospice facility.

Remembering loved ones….On Saturday, June 14th, 2014
Hospice of Southern Maine welcomed 150 guests to Gosnell
Memorial Hospice House to honor, remember and celebrate
the lives of their loved ones. A brick was placed in the terrace
overlooking the gardens for each loved one. Families and friends
searched for their loved one’s bricks and participated by telling
stories and paying tribute during the dedication ceremony.

Holding the light…Twilight in the Park, an annual
community event hosted by Hospice of Southern Maine,
celebrates and honors the special people in our lives. More
than 500 guests gathered at Deering Oak Park in Portland to
light more than 2,000 luminaria, each one dedicated in honor
or in memory of someone in our community. At the end of
the celebration, a paper lantern was lit and sent afloat with all
the names of patients who died in our care in the past year.

Annual Meeting
The voting of new Board Members… The 2014 Meeting
of the Members of the Corporation saw the addition of newly
elected board members Gayle Brazeau, PhD (University of New
England College of Pharmacy), the Honorable Barry Hobbins,
Esq. (Barry J. Hobbins Law Offices), Theresa Desfosses (State
Manufactured Homes), and Paul Wolf (Kennebunk Investment
Services). Retiring members David Perkins and Robert Schools
were recognized for their dedicated years of service. Daryl Cady,
CEO and Miriam Leonard, HSM Board Chair presented Elaine
Brady the Katherine L. Pope Leadership Award for her service as
interim Chief Executive Officer. Peggy Wescott, whose husband
Paul died in our care, was the guest speaker. She made us laugh,
made us cry and reminded us how important our work is to the
families and patients we serve.

COMMUNITY EDUCATION
Anne L. Hunter Memorial Thresholds Conference
Community Education about spirituality and
end of life… Dr. Kathleen Rusnak was the keynote
speaker for the 5th Anne L. Hunter Memorial Threshold
Conference. More than 200 hospice workers, clergy,
physicians, spiritual leaders and community caregivers
gathered for a day of education about spiritual issues
encountered at end of life.
Inaugural poet Richard Blanco closed out the day by
leading several exercises in exploring the empathic
power of storytelling as a tool for relating to those in
our care, and others that we touch every day. Local
musicians Jennifer Comeau, Todd Glacy and The
Drummers of Now provided the musical interludes.

UNE Hospice Immersion Experience
In-depth educational experience… Hospice of Southern Maine and University of New England’s College of
Osteopathic Medicine have joined together to initiate a new educational opportunity allowing UNE students a unique
experience in caring for patients during end of life. Pairs of students immerse themselves for 48 hours in the daily living
and working with terminally ill patients at Gosnell Memorial Hospice House. This educational experience allows
students to have hands on experience with end-of-life issues and how it is experienced by families and patients. The
students are exposed to the basic principle of hospice “Compassion, Care and Comfort.”

COMMUNITY OUTREACH
Community education… Hospice of Southern Maine’s
Community Outreach program works to educate the
community and other health care providers on the
importance of hospice care. Our Community Liaisons
provide education about our programs, services and
qualifying criteria for admission. Our collaborative efforts
with other health care providers and senior living facilities
ensures the best care for patients at end of life.
Outreach staff help to deliver timely and urgent information
so an admission is seamless. Staff help to answer questions
on the benefits of hospice care, so when it’s time to contact
Hospice of Southern Maine, patients and caregivers feel
knowledgeable. Often people tell us they wish they had
called us sooner, once they experience the comfort that
comes from having an experienced team there to help
understand the options.

In 2014, Hospice of Southern Maine partnered with the Maine Medical
Center’s Palliative Care Team and New England Cancer Specialists to
ensure specialized, trained physicians are available to our patients, seven
days a week. This unique partnership allows us to quickly respond to
emerging issues of care and provide continuity for many of our patients.
The new physician model gives access to a variety of expertise and
creates a greater team approach to patient care.
