SOLERA4ME LIFESTYLE CHANGE PROGRAM Frequently Asked Questions 1. What is the Solera4me Lifestyle Change Program? Also known as the Diabetes Prevention Program (DPP), the lifestyle change program helps participants lose weight, adopt healthy habits and significantly decrease their risk of developing type 2 diabetes. The program meets weekly for 16 weeks and then monthly for the balance of a year. The program teaches participants to make lasting changes by eating healthier, increasing physical activity, and managing the challenges that come with lifestyle change. 2. What’s included in the program? There are many versions of the lifestyle change program, but most include the following components: • • • • 16 weekly lessons, followed by monthly sessions for the rest of the year Lifesty le health coach to help set goals and keep participants on track Small group for support and encouragement Helpful tools, like wireless scales and fitness trackers. 3. Who is eligible for the program? The solera4me lifestyle change program is a new preventive benefit for Anthem Blue Cross of California m embers. 4. How do members find out if they qualify? Members should visit solera4me.com/lausd and take a 1-minute quiz to see if they qualify. All LAUSD employees and retirees w ho take the quiz, regardless of their plan coverage, will be entered to win one of 5,000 $10 Amazon.com gift cards. 5. If they’re qualified, how do Anthem members sign up for the lifestyle change program? Members should visit solera4me.com/lausd to learn more about the program and to sign up; or they can call 877–486–0141 to s ign up over the phone. 6. Is there a cost to Anthem members for participating? There is no cost to members. 7. Are all LAUSD employees and retirees who take the quiz, entered into the drawing for a n Amazon gift card? Yes, any LAUSD employee or retiree who takes the quiz and enters their information will be entered into the drawing. This includes any LAUSD employee or retiree regardless of plan coverage (i.e. they could have Anthem or they could have Kaiser Permanente). 8. Can a spouse and/or dependent of the LAUSD employee participate in the drawing? No, only LAUSD employees and retirees are eligible to participate in the drawing, and they must have an email address to participate as gift cards will be redeemed electronically. 9. Is there a limit of one gift card winner per household? There is no household limit; it’s possible that an employee and retiree in the same household could win, or two LAUSD partners. 10. When will the drawing take place? The gift card raffle will take place the week of April 17th, assuming all communications are sent on schedule. 11. Following the drawing, when should participants expect their gift cards? Gift cards will be fulfilled the week of May 1st. 12. How will the 5,000 winners be notified? Winners will be notified via email; non-winners will not be notified. 13. When can the participant redeem the gift card? The participant can redeem their gift card as soon as it is sent to them electronically. 14.
