This completed form must be provided at the time of re-enrolment. It includes two parts:

Registration Information for re-enrolling students for school year 2017-2018 (see below)

Personal & Billing Information for re-enrolling students (including application for DISCOUNT tuition fees
if applicable) (see reverse side of this form)

REGISTRATION INFORMATION:
Re-enrolling students are accepted in accordance with ISU’s most current registration information as provided by
parents in the spaces below. This form when completed below serves as the Contract of Enrolment between the
student’s parents and ISU for the academic year 2017-2018.
•
By re-enrolling my child in the International School of Ulaanbaatar, I acknowledge that I have completed
the Personal & Billing Information form (on reverse side) and will make payments as due according to the
payment schedule I have chosen (early; full; semester plan; quarter plan).
•
I understand that any late payments by me beyond the 30-day grace period will be subject to a late fee
equal to 1.5% per month of the balance due.
•
I agree to abide by the Articles of Association, Policies and By-laws of the Association of the International
School of Ulaanbaatar. I may request a copy of these policies, by-laws and articles at any time.
•
I understand that failure to abide by these Policies, By-laws and Articles of Association may result in the
loss of my right to remain a member of the Association and my child’s enrolment at ISU.
•
I understand that if any of the information that I have given here is false or misleading, ISU reserves the
right to reject the re-registration of my child, or, if my child has already been re-registered, ISU reserves
the right to cancel the re-registration.

 QUARTER PAYMENT PLAN for Tuition fees (4% surcharge for 4 equal payments: Aug 1, Nov 1 2017 / Feb 1, Apr 1 2018)
