Box Labeling Instructions for Shipment of a Class 6.2, Category A Shipment without Dry Ice
*Note: The box should be UN Package type approved for 6.2 and this must be designated on the box.
1. Shipper’s Information – Enter the complete address of the shipper including a phone number.
2. Consignee’s Information – Enter the complete delivery address of the consignee including a phone number.
P.O. BOXES ARE NOT ACCEPTABLE!
3. Name and Telephone Number of Person Responsible – Enter the name and telephone number of the
person responsible for the shipment.
4. Proper Shipping Names and UN Numbers – This is the Division 6.2 Proper Shipping Name. The Technical
Name does not need to be included but the UN number MUST be included. Please select the proper label
based on UN number description.

5. Hazard Label – Include the Division 6.2 (Infectious Substance) hazard shipping label, this label must be 100
x 100 mm for standard or large packages or 50 x 50 mm for small packages (dimensions of package can
only bear the smaller labels).
6. Orientation Label – This must appear on both (opposing) sides of the package.
7. Cargo Aircraft Only label – Required when the quantity of Category A substance exceeds 50 mL or 50g
(total for package). Must be 120x110 mm (or 60x55 mm for small packages). Must be an orange label with
black text.
