of your family doctor, dentist or Chinese medicine practitioner. It is
also your personal health record. Please keep it properly.
Please remember to bring along your health passport for every
health visits. This will help your family doctor, dentist, Chinese
medicine practitioner and other healthcare professionals
understand your health conditions and facilitate discussion of your
health promotion plan.
