the total cost. Add the postage and packing cost that applies to your order. Enter a value in the Donation
box if you wish to make an additional gift. Complete the Grand Total box. Please make cheques payable
to ‘Meningitis Trust Trading’ OR complete the credit/debit card section in full. Send your form and
payment to Meningitis Now, Unit 29, Penley Industrial Park, Penley, Wrexham, LL13 0LQ.
