Know Your Numbers FAQ Effective 03/01/2017; Updated 03/01/2017 Please see the Commitment to Health Policy for additional information. What is Know Your Numbers? As a member of the CarMax Medical Plan, CarMax wants you and your Spouse or Domestic Partner to commit to having an active role in your health. When you participate in our Commitment to Health Program, we ask you to take specific steps to understand your health risks. The Commitment to Health Program includes the “Know Your Numbers” events where you undergo specific Biometric Screenings (see below). It also includes the completion of an online Health Assessment on the CarMax Wellness portal. When you complete these two activities, you and your covered spouse or domestic partner will receive Medical Plan Credits on your bi-weekly paychecks. Please see the Commitment to Health Policy on the Benefits website for more information. Why does CarMax want me and my Spouse/Domestic Partner to know our numbers? CarMax believes that making a commitment to your health is important. Over the past few years we have integrated a focus on wellness into the CarMax culture. A key part of this is encouraging you to understand your health risks — to know your numbers. By participating in the Biometric Screenings and online Health Assessment, you’ll get important information about your health to help you make informed decisions and participate in programs to help you live a healthier lifestyle. We believe that healthy living is important for every family member. By knowing your numbers, you and your Spouse/Domestic Partner can work together to make positive changes to your lifestyle. What is measured in the Biometric Screenings? The following five Biometric Screenings are used to measure factors that contribute to Metabolic Syndrome: Waist Circumference Cholesterol Triglycerides Blood Pressure Glucose What is Metabolic Syndrome? The American Heart Association has identified recommended “healthy” ranges for each of the five risk factors that make up Metabolic Syndrome – the factors measured in your Biometric Screening. Being outside the recommended range for three or more of these risk factors represents an increased risk of heart disease and diabetes, which is referred to as Metabolic Syndrome. Approximately 34 percent of U.S. adults have the syndrome, according to the American Heart Association. For more information about Metabolic Syndrome, visit the American Heart Association website at http://www.heart.org. Where can I get my Biometric Screenings? There are three ways to get your free Biometric Screenings: Know Your Numbers Events — an on-site screening event held at every CarMax location in March 2017. Local Quest Diagnostics® Patient Service Centers (PSCs) — you and your Spouse/Domestic Partner can complete your screenings at a Quest Diagnostics PSC in your area. Schedule your screenings by calling (866) 908-9440 or My.QuestForHealth.com (Registration Key: carmax). You will need the associate’s ID number as well. Your Primary Care Physician (PCP) — you and your Spouse/Domestic Partner can submit your screening results when conducted by your doctor. This form is available on the CarMax Wellness portal, carmaxwellness.com, or by calling Preventure at (888) 321-4326. Do I need to do anything to prepare for the Biometric Screenings? For the best possible results, you should fast for 12 hours before getting your Biometric Screenings. However, fasting is not mandatory. No, participation in the Know Your Numbers activities is voluntary. However, you and your Spouse/Domestic Partner are only eligible to continue to receive a Medical Plan Credit (MPC) if you each complete the Biometric Screening and Health Assessment within 75 days of your coverage for this Plan Year becoming effective. If you are enrolled in the Medical Plan as of March 1, 2017, that means you need to complete your Screenings and Health Assessment by May 15, 2017. Does knowing my numbers take the place of a physical exam? No. It is important to have regular physical exams with your physician. The Biometric Screening and Health Assessment are not a complete picture of your health status. We encourage you to share the screening results with your doctor. What is the time frame for completing the Biometric Screenings and Health Assessment? For anyone enrolled as of March 1, 2017, the Biometric Screenings and Health Assessment must be completed between January 1, 2017 and May 15, 2017 (this is the “Know Your Numbers Completion Window”) to continue to receive the Medical Plan Credit (MPC) after May 31, 2017. If you enroll in coverage after March 1, you have a 75 day Grace Period from when your coverage becomes effective to complete the Screenings and Health Assessment. You will receive the Medical Plan Credit during the Grace Period. To maintain the Medical Plan Credit after your Grace Period, you must have completed the Biometric Screenings and Health Assessment within your Grace Period. What if either my Spouse/Domestic Partner or I am pregnant? If you or your Spouse/Domestic Partner is pregnant - in lieu of the Biometric Screenings - get a signed letter from your doctor verifying the pregnancy (regardless of trimester). Attach this to the Physician Screening Form found on the CarMax Wellness portal at carmaxwellness.com and submit the form to Preventure within 75 days of your enrollment in the Medical Plan in lieu of the Biometric Screenings. The online Health Assessment must still be completed by the deadline to receive the Medical Plan Credit. How will I know what my Biometric Screening results mean? Screeners at the Know Your Numbers events and at Quest Diagnostics PSCs will provide you with information explaining your Biometric Screening results. If you have additional questions about your results, please contact your doctor. Once you enter your Biometric Screening results in the online Health Assessment, you will receive a Personal Health Report on your personal, private dashboard on the CarMax Wellness portal. The Personal Health Report will contain information about your health risks along with recommendations about how to make positive changes, if any. What is the Health Assessment and who needs to complete it? The Health Assessment is a confidential online questionnaire on the CarMax Wellness portal (carmaxwellness.com) that asks questions about your everyday lifestyle - including your Biometric Screening results - and uses that information to provide a snapshot of your current health status and/or risks. The Health Assessment is located on the CarMax Wellness portal (carmaxwellness.com) and will take about 15-20 minutes to complete. If you are a new user: Visit carmaxwellness.com. Your username is your 6-digit Associate ID number For covered spouses and domestic partners, your user name is the 6-digit Associate ID number of the Associate that is the primary medical carrier plus the 4-digit month and day of your date of birth (Example: 0012340121 if your DOB is January 21). For associates and covered spouses and domestic partners, your password (until you change it) is KMX followed by your 8-digit birthdate: MMDDYYYY (Example: KMX01211970, if your birthday is January 21, 1970) Once registered on the site, click the menu in the upper left hand corner, then choose the “Take Health Assessment” from the drop down menu. Agree to the Terms of Use and click the “Next” button to be taken to the beginning of the assessment. If you have registered on carmaxwellness.com previously: Log in then Click the menu in the upper left hand corner, then choose the “Take Health Assessment” from the drop down menu. Agree to the Terms of Use and click the “Next” button to get taken to the beginning of the assessment. If you and/or your Spouse/Domestic Partner participate in the Commitment to Health, you must each complete the Health Assessment and Biometric Screenings by May 15 to receive the Medical Plan Credit after June 1. Completing the Health Assessment will give you an overall look at your habits and health risks, if any. It will also provide information on how to improve any potential health risks you may have. If you participate in the Commitment to Health, you must complete the Health Assessment and Biometric Screenings within the Know Your Numbers completion window to receive the Medical Plan Credit after June 1. What should I expect when I take the Health Assessment? Have your most recent Biometric Screening results handy to answer the related questions. It will take approximately 15-20 minutes to complete the full Health Assessment. What if I do not complete the Biometric Screenings and Health Assessment within the stipulated time frame? Completing the Biometric Screening and Health Assessment are required steps for you to receive a Medical Plan Credit. If these activities are not completed within the first 75 days of coverage (by May 15, 2017 for those enrolled at the start of the Plan Year), you will forfeit the Medical Plan Credit at the end of your Know Your Numbers completion window and not be eligible to receive it again until June 1, 2018 (provided you complete the Biometric Screening and online Health Assessment in the next Plan Year’s Know Your Numbers completion window). What happens after I complete the Health Assessment? You will receive a Personal Health Report on your personal, private dashboard on the CarMax Wellness portal. The Personal Health Report will contain information about your health risks along with recommendations about how to make positive changes, if any. Are my results from the Biometric Screenings and Health Assessment confidential? Yes. All individual screening results are completely confidential and never shared with CarMax. CarMax receives aggregate information - but never any individual information - about the health status of participants. Our program administrator, Preventure, will receive your personal health information and use it to identify the programs that are right for you and help you achieve your health goals. What will Preventure do with my results? Preventure will use your results to identify any health risks and recommend free programs designed to reduce those risks and help you achieve personal health and wellness goals. Once Preventure receives your Biometric Screening and Health Assessment results, they will determine if you have met the requirements to maintain the Medical Plan Credit. What else do I need to do after I complete the Biometric Screenings and Health Assessment? You do not need to do anything further after you have completed your biometric screening and online health assessment. CarMax no longer requires participation in Healthy Lifestyle Coaching in order to maintain medical plan credits.
