Parish Nurse
Services
The Parish Nursing Program promotes health
and wellness for seniors and vulnerable
populations in our community, including
homeless individuals and families, people who
lack access to health care and disenfranchised
veterans. Directing people to appropriate
providers, linking them to primary medical
homes and identifying appropriate resources
to meet their health needs are part of our
Parish Nurses’ responsibilities. Through
partnerships with community agencies, clinics
and churches, the program also provides
opportunities to socialize, be with peers and
improve family relationships. Services include
health education and emotional support,
health care management, health assessments,
observation and referrals into care. Services
are provided at four church locations and a
homeless facility.
In fiscal year 2014, we provided Parish Nurse
services to 5,326 clients. We are currently
expanding this program and are seeking
partnerships with nurses who provide similar
services in their faith communities.

Mobile
Health Unit
Program
The Mobile Health Unit has been providing
health care in our community since 2009
and is dedicated to expanding access
to health services for the homeless and
minority populations in targeted areas.
Services onboard the van include:
» Rapid HIV and OraSure testing
» Rapid hepatitis C testing
» Health exams by a doctor or an advanced
registered nurse practitioner (ARNP)
» Health screenings (cholesterol, diabetes,
blood pressure and pregnancy)
» Care management
» Links to health care services and
affordable health care navigators
» Childhood immunizations
» Flu vaccine shots
The Mobile Health Unit operates up to
six days per week. In fiscal year 2014,
it served 2,497 people.

Growing
Healthy Kids
Growing Healthy Kids is dedicated to improving
knowledge, attitudes and behaviors toward
good nutrition, fitness and self-image. Its
programs target underserved, lower-income,
at-risk elementary school students in parochial
and public schools. We partnered with
community organizations such as the 4-H Club,
Florida Agriculture in the Classroom and the
Florida Farm Bureau. As a result, 159 classroom
trainings and healthy lifestyle tools were
provided to 391 teachers and 3,745 students
and parents in fiscal year 2014.
The Pilates for Girls program aims to improve the
mental and physical health of female students
and their teachers. MELT (myofascial energetic
length technique) classes were also provided
to 274 school faculty members, parents and
seniors. MELT enhances body awareness,
rehydrates connective tissue, decreases stuck
stress that accumulates from daily living and
returns the body to a more ideal state.
Growing Healthy Kids also guided schools in
planting 83 on-site produce gardens that were
cultivated and harvested by students. This
experience gave students, their families and
school communities new attitudes toward
food, nutrition, the earth, green programs and
sustainability.

Homeless
Services and
Hunger Initiatives
We participate in Broward County’s annual Point-inTime (PIT)—a reliable count of people experiencing
homelessness in our community. The data is
used by government agencies to properly allocate
assistance funds. The 2014 PIT count identified
2,766 homeless individuals.
Our response to our community’s homelessness
issues included:
» Placing a Parish Nurse at a homeless day shelter
and at church-based homeless feeding programs
» Providing outreach screening services: breast
health, blood pressure, blood sugar, cholesterol,
and HIV and hepatitis testing (as well as
assistance with necessary follow-up services)
» Partnering with Hope South Florida to create a
Wellness Center with on-site health care services
and education for homeless families
» Joining homeless agency boards of directors
» Serving as a leading community partner on the
South Florida Hunger Coalition—a resource for
raising food, funds and community awareness
» Having Holy Cross Associates donate and
distribute food to small food pantries in Broward
County
In fiscal year 2014, our homeless services and
hunger initiatives also provided:
» Services to 2,177 homeless individuals
» More than 3 tons of food to Broward’s food pantries

School
Health
Program
The School Health Program gives thousands
of students access to school-based vision,
hearing, immunization and scoliosis
screenings, as well as health education
classes and immunizations. In addition,
classes on CPR, blood-borne pathogens and
student-related health concerns (diabetes and
HIV/AIDS, for example) are available to school
personnel. The program also provides health
education in nontraditional school settings,
such as the Broward County Juvenile
Detention Center.
One of the goals of the School Health
Program is to raise the immunization rates
of school-age children in our community.
In addition to providing immunizations from
the Centers for Disease Control’s Vaccines
for Children initiative to eligible children,
members of the program actively participate
in Broward’s Immunization Coalition.
In fiscal year 2014, the School Health
Program provided:
» 5,841 screenings
» 1,990 immunization reviews
» 4 82 immunizations
» 491 referrals for follow-up
» 88 health education classes
(1,065 students and 428 adults)

Goodstart
Program
The Goodstart Program provides health
education and promotes wellness to
at-risk preschool students at their
daycare facilities. In addition, monthly
caregiver workshops educate providers
on children’s health issues. At these
workshops, caregivers obtain the
necessary credits to stay licensed and
remain in good standing
for employment.
Partnering with Broward Impact,
Goodstart presents monthly training
for seniors working with young children.
In addition, a partnership with the Jan
Moran Collier City Library promotes
health literacy through a monthly
book club. The combined efforts of
this multigenerational program yields
thousands of hours of essential health
and wellness education.
In fiscal year 2014, the Goodstart
Program served:

» 261 school-age children
» 924 caregivers
» 147 seniors

Partners in
Breast Health
The Partners in Breast Health (PIBH) program
aims to increase access to breast health services
in underserved communities throughout Broward
County. The goal is to save lives by reducing barriers
and providing education and early detection.
Partner community health agencies, federally
qualified health centers and outreach workers give
referrals to the program, and women are assessed
for program eligibility. At an appointment, a physician
and/or an advanced registered nurse practitioner
(ARNP) may provide one-on-one education, general
health assessments, a clinical breast exam and
a mammogram referral for women ages 40 and
older (or for younger women with symptoms of
breast cancer). Follow-up care assists patients with
diagnostics, care and treatment. Special needs are
often met via the hospital’s financial assistance
program. PIBH receives private and grant funding for
screening and diagnostic services.
In fiscal year 2014, PIBH provided
1,253 screenings and diagnostic services
(mammograms, ultrasounds, magnetic resonance
imaging [MRI] tests and biopsies).

Diabetes:
Chronic Disease
Management
Encouraging individuals and their health care
providers to use preventive health services, such
as disease self-management, chronic disease
management, and participation in primary medical
homes are key components in helping our community
remain healthy.
During this past year and a half, Holy Cross Hospital
had a dedicated nurse provide outreach education
to clients with diabetes in our community. A lack of
understanding about diabetes (and related diagnoses)
often contributes to hospital admission, readmissions
and frequent visits to the emergency room. The
Diabetic Nurse Educator gives health information to
people with diabetes, their families and caregivers,
paying special attention to health literacy and cultural
and linguistic appropriateness. Understanding how
to take control of diabetes empowers our community
members to take control of their health and future.
In fiscal year 2014, our diabetes care included:

» Group education: 74 people
» Blood sugar, total blood cholesterol and

HIV and
Hepatitis C
Testing
Since 2008, Holy Cross Hospital has
provided easy access to rapid HIV
testing. In 2014, we were able to add
access to rapid hepatitis C testing
for our community. Testing services
are located at targeted outreach sites
and aboard the Mobile Health Unit.
All clients are given pre- and posttest counseling, and those people
confirmed to be positive are directed
to appropriate care and treatment. In
addition to the HIV and hepatitis tests,
clients are offered blood pressure, blood
glucose and cholesterol screenings.
In fiscal year 2014, Holy Cross provided
HIV testing services for 8,791 people
and performed 3,938 HIV tests. The
positivity rate was 0.2 percent.
We also performed 551 hepatitis C tests,
and the positivity rate was 3 percent.
