Guidance for applicants to the North East and Cumbria Dementia Fund Please note that the responses to some of the questions in this form have character limits; these limits are noted below the relevant questions in the application form. You should complete the form using a word processor (such as Microsoft Word) and save it in Word document (.doc) or Rich Text Format (.rtf). Please do not email it as a PDF. You should save the form regularly while you are completing it. You must submit this form by email to [email protected] by 7th July 2014. To make sure you supply us with the answers to all required questions you should complete the questions and sections in order. You should answer each question carefully and be sure the answers you give actually address the key points these questions are seeking to draw out from your response. Date your organisation was established 1 Subsection Question Guidance 1.4 Tell us about your organisation’s aims and the reason for setting it up. You can find more information about which organisations are eligible to apply for our UK Grants on our policies page. Organisation purpose What is the purpose of your organisation and what are its main activities? 1.5 Leadership of your organisation Do the majority of people on your board have first-hand experience of the issues you are tackling? 1.6 1.7 We ask this question so we can see how many user led organisations are applying to the Fund. You will not be excluded on the basis of the answer to this question. By user led we mean those organisations which have users at their heart, most usually where they make up a majority of the board. Where this is not the case, you would need to be able to show how users shape, influence and direct the organisation. User involvement How does your organisation consult or involve the people who use the services it provides? How has this been done for this project? We ask this question so we can see how you as an organisation involve and consult with the people who use your services. Organisation finances By free reserves we mean those reserves (savings) that are unrestricted – that your organisation is free to spend to further your objectives. Free reserves should include designated reserves which trustees have earmarked for a particular use without committing the funds legally. Some reserves have restrictions; for instance if you have been given a grant to deliver specific activity and you have not spent it all, this cannot be spent 'freely' and we would not expect it to be included in your free reserves. The date your organisation's current financial year will end Please provide the following figures for your organisation's most recent full financial year (income, expenditure, total reserves, free reserves) We are especially interested in how you have consulted and involved people with dementia and their Carers in developing this project. You can find more information about our policy on reserves on our policies page. 2 Subsection Question Guidance 1.8 If you have received a grant from Comic Relief, Northern Rock Foundation or Ballinger Trusts previously you should be able to find the reference for this on correspondence relating to the funding. Please provide a brief title for your proposal, excluding your organisation name 2.2 Please tell us where the work will take Please specify postcodes which best reflect the locations and characteristics of the areas 3 Subsection Question place Is the exact location of the work confidential? Please provide the postcode for up to four main work locations 2.3 Partnership If you are delivering this specific piece of work with other organisations, please tell us who they are and what their role is. 2.4 The need for the work What need do you want to address and why is it important? 2.5 Guidance you work in. This should be the full postcode. If your project delivers work across a locality, city or county, please provide postcodes within these which best represents the range of locations you work across. We use this information to understand the characteristics of the locations where you are working, such as whether it is urban or rural, its level of deprivation or where there are gaps in similar services. We also review the locations of all our applicants to understand where we are attracting applications from. You do not need to answer this question if all aspects of this project are delivered online. We welcome applications from partnerships led by an eligible not for profit organisation. Please tell us why you have chosen these partners what role they will play in the project and if this is a new partnership or you have worked together before. You should demonstrate how you have identified the need for the project you are proposing for the particular group of people/organisations who would benefit and why this need should be addressed (citing independent research, your own data, information from users or others in the field). The difference the work will make What difference will the work make to the people you are supporting and/or to your organisation? We want to know in what way the activities or service you want to provide will bring about change for the better in the lives of the people who use your project. What change are you aiming to bring about for them and/ or to your organisation? 4 Subsection Question Guidance 2.6 If you are providing direct services to people, please tell us how many people you estimate you will work with over the life of the grant. If you are doing any other type of work – such as awareness raising or campaigning – please provide a broad estimate of the number of people you think will benefit. If your proposal covers both of these approaches, please only include the figures for direct service delivery. People who will benefit How many people do you expect to benefit from the work throughout the duration of the grant? 2.7 Activities What activities will you carry out and why do you think these will be effective? Which elements of the work do you want the grant to fund? e.g. salaries, project running costs 2.8 Relevant experience What is your organisation's relevant experience in this type of work? Tell us about the activities you will provide and why you think these are the right activities and that they will bring about the changes and difference you identified in question 2.5. Tell us what you would like to use the grant for this could be project costs, salaries and a contribution to your overheads. We will not fund capital projects but will fund some small capital items that are integral to the project. Have you carried out any similar work before and/or worked with this client group? If so, how long for? Please tell us about any relevant organisations or networks you will be working with/consulting on the project. 2.9 Monitoring and Evaluation How will you monitor and evaluate your work? We want to know how you will know that the difference you want your project to make is happening? How will you capture and record this information and how will you share the learning from your project? 2.9.1 Proposal summary A clear 2-3 sentence project description that we can use to describe your project on our website and to other people. We do not need a detailed list of project activities in this question or a description of the context. Please provide a brief summary of your proposal 5 Subsection Question Section 3 - Finances Subsection Question 3.1 Finances How long would you like the grant to last? Guidance Guidance We use this information to see whether your organisation’s finances are healthy and to see how much money you are used to managing. Grants can last between 1 and 3 years and can range from below £2,000 to up to £200,000. Total cost of the work How much are you applying for from the Fund? How much are you seeking from other funders? How much of the amount that you are seeking from other funders is already secured? Own contribution Section 4 - Contact details Subsection Question 4.1 Your contact details Guidance Please give the contact details of the person who we can contact if we have any questions about the application and if you are shortlisted we will contact to arrange the 6 Subsection Question Section 5 – Declaration Subsection Question 5.1 Declaration Guidance assessment. This will take place in July/ August 2014 so they MUST be available at this time. Guidance Please confirm that you are authorised to submit the application form on behalf of your organisation. Don’t forget to attach your supporting documents. If you are applying for a grant of less than £2,000 please provide: Budget for the project, copy of a relevant job description and person specification if you are applying for staff salaries and a signed copy of your most recent financial statements. If you are applying for more than £2,000 please provide: Budget for the project, your organisation’s current year’s budget, your most recent management accounts, a signed copy of your most recent audited or independently examined accounts, your safeguarding policy and a copy of your governing documents. 7
