Utilities Board Sylacauga Gift Card Drawing
October – December 2010
No purchase necessary to enter
The winner must be a current water, electric, or gas utility customer
during the month of the drawing and listed as “active” in the accounting
department with the Utilities Board of the City of Sylacauga, AL.
Pre-printed “Entry Forms” will be available at the Utilities Board Billing Office
located at 301 North Elm Avenue in Sylacauga beginning October 01, 2010.
Entry Forms will be inserted in the Customers Utility Billing Invoice,
beginning with the first billing cycle for October, 2010 and thereafter
through typical billing cycles each month through December.

Entry data must match a current utility customer of the Utilities Board of the City of
Sylacauga with a valid “active” account number.
Customer must populate (complete) the entire entry form with correct customer
account information and telephone numbers associated with their account to qualify
for a prize.
Verification of the telephone numbers submitted may obtained by the employees of
the Utilities Board Sylacauga, AL
Employees of the Utilities Board and their immediate family members are not eligible
to win.
You must be 18 years or older to enter.
Customers may enter one entry form provided by the Utilities Board once a
month for the months of October through December, 2010
An entry form may not be reproduced or duplicated by any means and
used as an entry. If it is determined that a copy has been made, the entry
will be disqualified.
If more than one entry per customer per month is verified, the customer
may be disqualified.
The Utilities Board cannot be held responsible for any problems,
misunderstandings, inconvenience associated with the misprint or loss of
an entry form. The Utilities Board cannot be responsible in the event of a
disaster, employee error, or computer error pertaining to the drawing.

Prizes will be a gift card equal to ($50.00 USD) to be redeemed at the
business represented on the card. Or where the card was purchased as
indicated clearly on the gift card.
Entries will be collected and placed into a secure container until time for
drawing. The entries will be thoroughly mixed and thereafter one entry will
be randomly drawn by a neutral party and given to Utilities Board
personnel to complete the agreement between winner and sponsor
Every effort will be made to contact the winner on the day of the drawing.
To receive the gift card, the winner will need to come to the Utilities Board
Office location at 301 North Elm Avenue, or provide a mailing address and
have the card mailed to their home or business.
If the winner cannot be contacted or verified with the information provided
on the entry form, another entry will be drawn on the same day (until end
of business day) or until a winner can be contacted that day and verified.
The winner’s primary number or cell number will be used as the contact
number for the winner and once the Name, Account, and Address have
been verified, the entry will be declared the “winner” for the month.
Winners names will be posted on the Utilities Board Web Site at
http://www.sylacauga.net

By submitting an entry form, the entrant releases the Utilities Board from
liability in connection with the use of any item purchased with the gift card.
By submitting and entry form, the entrant permits the Utilities Board to use
the winner’s name only to promote the drawing where permitted by law.
The value of the prize may be viewed as taxable by the City of Sylacauga,
Alabama, State of Alabama, and United States Federal taxes at the time of
redemption.
All prizes will be awarded as one gift card valued at $50.00,
There will be no substitutions for the prizes listed above for any reason.
The Utilities Board is the sole sponsor of the drawing or promotion. The
“$50.00 value gift card” awarded as the winning prize will be purchased by
the sponsor from an established local business who will in turn honor the
card in Sylacauga Alabama, or at another location of the same business or
company. Any expiration dates applied by the business to the card is the
responsibility of the winner to verify and to redeem the card prior to the
expiration date.
The intent for this drawing is to collect Utilities Board customer
information for Utility Board use only. Telephone numbers, street
addresses, email addresses, and account information will not be passed
on to any other party for any reason. Upon data entry into the Utilities
Board Accounting and OMS (Operations Management System) Systems,
the entry forms will be destroyed or shredded.
A list of official rules and the winners will be posted on the Utilities Board
