The following features are available when your company offers

Critical Illness
Insurance1
• Covered employees are eligible for a Health Screening Benefit2 that puts money
back in their pocket after having standard screening test performed, including annual
physical exams, mammography and chest X-rays.
Dental
• Dental benefits play an important role in employees managing their health and
creating a personal safety net. MetLife offers a variety of online tools for employees
to make the most of their coverage:
- The Oral Health Library contains articles and tools that can help employees learn
more about the importance of oral health and its connection to overall health.
- The MetLife Dental Health ManagerSM,3 provides tangible, actionable insights
that can help employers make better benefit decisions, and can help your
employees manage their oral and overall health.
- Interactive Dental Risk Assessment Tools to help employees determine their risk
for periodontal disease or tooth decay to then discuss the results with their dentist.
Disability Insurance
MetLife’s Disability Health and Wellness Connection4 works with your existing
medical carrier(s), wellness provider(s) and disease management programs to drive
employee awareness and utilization of these programs and help you and your
employees get the most from your overall benefits investment. Health and Wellness
Connection is aimed at providing:
- Coordination between programs to allow appropriate and timely intervention in
chronic condition and disease management
- Improved health status of employees
- Ability to identify trends and top drivers impacting absenteeism and health
plan costs

coverage, employees have access to an Employee Assistance Program5 (EAP) to

Financial Education
Series
PlanSmart provides employees with access to a range of financial and retirement
education resources through on-site workshops, optional personal consultations,
and decision-support assistance. The educational series offers employees the
information and tools they need to take concrete steps toward achieving their
financial and retirement goals.
PlanSmart’s holistic approach connects the workplace benefits you offer with your
employees’ overall financial strategy, helping to increase benefits utilization,
appreciation, and overall loyalty.
Vision10
• Eye exams are more than the first step to correcting vision problems or treating eye
diseases. They can also help detect and monitor serious health problems like diabetes.
That’s why MetLife Vision benefits include the Diabetes Awareness Program as a
standard feature. The program provides outreach materials to help educate members
with diabetes about the importance of regular eye exams. In addition, registered
program participants will have access to our Vision Health Library that consists of
educational articles and resources (Coming Soon in 2013).

most group accident and health insurance policies, MetLife’s CII policies contain certain exclusions,
limitations and terms for keeping them in force. Product features and availability vary by state. There is
a preexisting condition exclusion in all states. There is a benefit suspension period between recurrences
of the same covered condition. A more detailed description of the benefits, limitations, and exclusions

2. There is a $50 benefit with a $15,000 Initial Benefit Amount and a $100 Benefit with a $30,000 Initial
Benefit Amount. In most states there is a 30 day waiting period for the Health Screening Benefit.
There is no waiting period for MD sitused cases. The Health Screening Benefit is not available to NH
sitused cases or NH residents. There is a separate mammogram benefit for MT residents and for cases
sitused in CA and MT.
3. Available for groups with 1,000 or more eligible employees, excluding Full Service Dental for Retirees
and Dental HMO/Managed Care benefit plans. Other restrictions may apply. Please contact MetLife for
complete details.
4. Available for employers with 3,000+ covered employee lives. MetLife does not provide any Disease
Management, Health and Wellness Programs or Data Warehousing. Employers will need to contract
for these services outside of their relationship with MetLife.
8. Subject to state law, and/or group policyholder requests, the Total Control Account (TCA) is provided
for all Life and AD&D benefits of $5,000 or more. The TCA is not insured by the Federal Deposit
Insurance Corporation or any government agency. The assets backing TCAs are maintained in
MetLife’s general account and are subject to MetLife’s creditors. MetLife bears the investment risk of
the assets backing the TCAs, and expects to receive a profit. Regardless of the investment experience
of such assets, the interest credited to TCAs will never fall below the guaranteed minimum rate.
