Thank you for submitting your athlete’s nomination for the 2015-2016 NSCAA High School Scholar AllAmerica Team. Please follow the directions below to complete the nomination.
1. Obtain the necessary signatures on the statement below granting permission to the NSCAA to use their
information in new releases regarding the reward.
2. Submit either a certified copy of the student’s transcript or a copy of the transcript signed by the school
principle or guidance counselor to the address below.
STATEMENT OF PERMISSION
The undersigned hereby grants the National Soccer Coaches Association of America (NSCAA) permission
to include information submitted as part of the nomination process for the NSCAA Scholar All-America
Team/Scholar Athlete of the Year awards program in any publicity vehicles the NSCAA deems appropriate.
This includes, but is not limited to, news releases disseminated by the NSCAA to members of the media
and through its official website.
