This editor is loaded and hosted by InScribe but is not directly part of the InScribe program. This guide contains instructions for using the AnyModal editor with the Emdat InScribe application. From the MT perspective, the AnyModal editor will be used to create transcription from dictation. This editor has features designed to remove some of the clerical responsibilities associated with the transcription process. For example, it is unnecessary to apply style formatting to the section headings such as bolding, or have to remember that Client ABC wants text placed two spaces after the section heading while Client XYZ wants their text to start under the section heading. The ShadowScribe system performs these tasks automatically when the job is delivered. The editor can also help improve typing efficiency through the use of shortcut keys and for those clients that use standardized sections, a drop down list of predefined sections can be used to insert a section without having to type out the full name. NOTE: This guide assumes your shortcut profile is setup to use the VR-Enhanced Shortcuts and will refer to these shortcuts throughout. To make sure you are using these shortcuts go to the Options Menu, select InScribe shortcut keys, and make sure the VR: Use VR-Enhanced shortcuts is checked. Emdat Proprietary Page 2 AnyModal Editor for the MT Guide Brief Overview of How ShadowScribe Works When a clinician begins dictating on ShadowScribe, the system starts building a speech profile in a training mode. This profile contains information on sound quality of the audio file, sound quality on the speaking environment, and the unique speaking patterns of the clinician. Each time the clinician dictates, this information is processed and added to the profile. After a period of time the profile is completed. When this occurs the system is ready for speech recognition and dictations are transcribed as “draft documents” for an Editor to correct. The amount of time this process takes depends on clinician dictation minutes and number of jobs processed. On average, a clinician can be trained with as little as 1 – 2 hours of dictation. NOTE: Not all dictations will necessarily qualify to become draft documents. Some clinicians’ jobs, even if they are fully trained, can still go to transcription. This can happen if the ShadowScribe system determines it cannot transcribe the dictation. For example, someone different could accidently be using the clinician’s ID for their dictation, or the clinician could have a cold, or, or speaking in an excessive noisy environment, etc. Dictations can also go to transcription if they do not meet the client acceptable draft quality threshold. This threshold determines whether or not a draft is acceptable for editing. From a typing only perspective, you will not have to worry about draft documents. At this stage, all of the work will be typed from scratch but it is good to know how the system works since most MTs that type will eventually edit. Transcribing in the AnyModal Editor The AnyModal Editor, also known as the ShadowScribe editor, is the tool used to create transcriptions for the ShadowScribe system and the client. It is a separate editor that is integrated into InScribe and will load when a ShadowScribe dictation loads. InScribe will switch between the Emdat Editor and the AnyModal Editor depending on clinician setup in the Emdat system. If the clinician has been setup for ShadowScribe then the AnyModal Editor will load otherwise the regular editor will load. ShadowScribe Menu Transcription area When a ShadowScribe job is loaded, all major InScribe functionality will remain the same with a few changes. First, you will notice the editor itself looks nothing like the InScribe editor. There are no toolbars or formatting options available. Text is typed into the transcription area illustrated above. As you type, the white space of the transcription area will expand to make room for the text. Another noticeable change is that most jobs will not have document templates. This is actually preferred because the system learns best as it analyzes transcription typed from scratch. Your account manger will determine whether or not a document template will be used. As you transcribe, you will add sections using Ctrl+N and either type out the section title manually or use a section drop down list to pick the appropriate section as it is spoken. Manually typing it or selecting it from a list will depend on the client’s requirement. Finally, the ShadowScribe menu has additional features for the editor and will only appear when a ShadowScribe job is selected in your dictation queue. NOTE: Because there are no tool bars, bold, underline, and italics are not available as icons. However, these functions are available under the format menu or from these shortcuts: bold (Shift+Ctrl+B), Italics (Shift+Ctrl+I) “I” as in the letter “i”, Underline (Shift+Ctrl+U). The Transcription Process Standard vs. Verbatim Sections As with regular transcription, you will playback the dictation, fill in patient demographics, and begin typing as the clinician speaks. When a section such as “History of Present Illness” is spoken, this is your cue to insert a section by using the Ctrl+N key combination. After typing a few letters a drop down may or may not appear. Whether or not a drop down list appears depends on the client requirement to either have a standardized setup or an as dictated verbatim setup. In a standardized setup, the client requires specific standard section titles to appear in the final document regardless of how they were dictated. For example, if the client standard is to always display the “History of Present Illness” section and Dr. A dictates “History” and Dr. B dictates “HPI”, because of the standard both doctors will always see “History of Present Illness” in their final document. So your account manager would setup a list of standard titles that would appear in a drop down list when you did Ctrl+N. In contrast, in an as dictated verbatim setup, the client has strict verbatim requirements. If Dr. A dictates “History” and Dr. B dictates “HPI”, this is what should be returned in the final report. In this case you still need to do Ctrl+N but you would not see a drop down list, you would just see the section placement marker (see next section for more info) and would manually type out “History” for Dr. A and “HPI” for Dr.B. Whether you have a standard or as dictated verbatim setup, after you insert a section you will always type the content below the section title, never type content within the section title as this will cause formatting problems. The same setup is true for subsections as well. If standardized, the clinician speaks a section followed by a subsection, such as “Physical Exam” followed by “Vital Signs”, this is your cue to insert a section using the Ctrl+N key combination and then a subsection using the Shift+Ctrl+N key combination. Just like with sections, a pop-up list may appear with all the possible subsection names to insert. In contrast if setup is as dictated verbatim, with subsections you’ll do the same thing but will not get any drop down list and will have to manually type out the subsection title. Your account manager will decide the setup either standardized or as dictated verbatim from the client requirements. In some verbatim cases, it may be possible to setup a partial list of the most common section titles that all dictators speak. So you would get a drop down list of some titles. Your account manager will decide if this is appropriate. Emdat Proprietary Page 4 AnyModal Editor for the MT Guide Using Section and Subsection headings lists Section and Subsection headings are a quick way to insert a dictated section title into the transcription. When used with a standardized workflow, they save you time and improve efficiency because you do not need to type out the whole heading. In addition, they do not have to be formatted a certain way. For example, if the client requires all section titles to appear in bold and upper case this formatting will be automatically applied at time of delivery. For more information see The Transcription Formatting Rules section of this document. The illustration below details three main items of the section and subsection heading tool. Section Heading Place Marker Transcription area Section/Subsection heading list list 1. Section Heading Place Marker. This is the area where the section heading will be inserted after making a selection from the drop down list. Only the section heading should be inserted here. You should not typethe content of the section here. 2. Transcription area. This is where you type the content of the section. Text will always appear below the section heading. 3. Section/Subsection heading list. This is list of predefined sections for the current document type. This list is setup by your account manager. The numbers to the right of the name are an internal code for the ShadowScribe system and can be ignored. If you do not see a list, then your client likely has an as dictated verbatim setup. Inserting a Section Heading 1. Place your cursor in the body of the transcription where you want to insert a section heading. 2. Use the Ctrl+N key combination to make the section heading list appear. 3. Select an item by typing the first few letters of the heading. As you type, the word that most closely matches will highlight. Alternatively, you could also use your mouse to make a selection; however, using the mouse is discouraged because it is not efficient. 4. To insert the heading, hit the tab key. Your cursor will be immediately placed below the section heading where you can begin typing. Emdat Proprietary Page 5 AnyModal Editor for the MT Guide Note: if you do not have a list to choose from, you will need to type out the title manually, this is true of subsections also. Tip: if you have a client that doesn’t use the drop down list, you can add some of the most common sections as Autotext. Inserting a Subsection Heading 1. Place your cursor in the body of the transcription where you want to insert the subsection heading. 2. Use the Shift+Ctrl+N key combination to make the subsection heading list appear. 3. Select an item by starting to type the first few letters of the subheading. As you type, the word that most closely matches will highlight. Alternatively, you could also use your mouse to make a selection; however, using the mouse is discouraged because it is not efficient. 5. To insert the subheading, hit the tab key. Your cursor will be immediately placed below the subsection heading where you can begin typing. Note: If you are using a subsection drop down list, that list will only appear under the section it was defined under. For example, if your account manager setup subsections under the Physical Exam and you do Shift+Ctrl+N then you’ll see the list. Otherwise, you will only see the subsection placement marker. Inserting a Red Section In some cases the clinician will speak a section or subsection that is not in the predefined section list. When this happens, you will need to manually add the section. When you add a section or subsection it will always appear in red indicating it is not part of the predefined section list. Make sure to always add the red section or subsection in initial caps E.g. “History” instead of “HISTORY”. This ensures the rendering program will choose the proper case when the job is delivered to the client. Note: Red sections and subsections only appear in a standardized setup. If your setup is a verbatim setup, section placement markers will always appear in purple. Emdat Proprietary Page 6 AnyModal Editor for the MT Guide Removing a Section 1. Place your cursor in the section or subsection you wish to remove. 2. Use the Ctrl+N key combination (Shift+Ctrl+N for subsection) to convert the section into text and then hit delete. Using lists Under certain section headings the dictator may require a list. When lists are added, they will always appear as bullet points. You should not remove the bullets and manually type numbers unless explicitly instructed by your account manager. The bullets will be converted into numbers at time of delivery or if you do a preview with template. For more information see The Transcription Formatting Rules section of this document. Inserting a List 1. Place your cursor under the appropriate section or subsection. 2. Use the Ctrl+L shortcut key to add a list item. As with any bulleted list, you can hit enter after adding the item to automatically start a new list item. You can also indent or unindent a list item by using the Tab or Shift+Tab key combination. Alternatively, you can use Ctrl+M to indent and Shift+Ctrl+M to unindent. Removing a List 1. Place your cursor on the line that has the bullet. 2. Use the Ctrl+L shortcut key to remove the list item. This will remove the bullet point from the sentence. Using Autotext Autotext is shared between both the regular Emdat and the AnyModal editor and works the same in both instances. Autotext should be used as an efficiency tool and should consist of words, phrases, sentences or paragraphs. Autotext that contains template formatting or numbered lists should not be used. This is not supported by the AnyModal editor and makes it very difficult for the ShadowScribe engine to learn from. If you have autotext that has either formatting or numbered lists, you should have your account manager make this into a ShadowScribe document or dictation template. Emdat Proprietary Page 7 AnyModal Editor for the MT Guide (Examples of autotext that should not be used) Should you choose to use this type of autotext, you will lose all formatting (such as bold, italics, underline, indents, alignment, and extra carriage returns) once it is expanded into the AnyModal editor. Transcription Formatting Rules Transcription formatting rules control the style formatting of the document, giving the document the client’s desired look. These rules apply to the structure items in the transcription which include sections, subsections, and numbered lists. When the rules are applied they control things such as bolding, casing, section content placing to name a few. Therefore, it is unnecessary to format any text, numbered lists, sections, or subsections. This frees you to focus on the quality and medical accuracy of the document without spending time on the formatting details for each client. For example, if the client requires all section titles to appear in bold and upper case this formatting will be automatically applied at time of delivery. This means if you add a section and it appears in sentence case, you do not have to retype it in upper case and bold it. When the job is delivered a predefined formatting template is applied to do this formatting for you. If you need to see what the transcription looks like with all formatting applied, you can use the Preview with Template Ctrl+Alt+B option under the InScribe file menu. Using ShadowScribe Document Templates Whether a document template is used or not is determined by your account manager. If document templates are used, they are typically used when the client always requires certain sections to be part of a dictator’s transcription whether they are dictated or not. In some cases, you may only see a partial document template. For example, the client might require only the “History of Present Illness” and “Medications” sections always be part of the transcription but the rest of the sections could vary. Emdat Proprietary Page 8 AnyModal Editor for the MT Guide Section heading Transcription area Subsection heading Transcription area for Subsection headings Similar to regular transcription, when a ShadowScribe job loads with a template it will automatically appear in the editor. As with any section or subsection, whether added by the MT or as part of a document template, you will always type text below the section into the transcription area. Using ShadowScribe Dictation Templates (or Normals) ShadowScribe dictation templates are used the same way regular dictation templates are used and have the same purpose. Unlike regular dictation templates, ShadowScribe dictation templates can have two formats. One format will look like canned text such as a few sentences or paragraphs lumped together. The other format looks similar to ShadowScribe document templates in that they appear as section and subsection headings when inserted. Inserting a ShadowScribe Dictation Template 1. Place your cursor in the appropriate section or subsection. 2. Use the Ctrl+Alt+G key combination to make the Dictation Templates dialog appear. 3. Select a ShadowScribe template from the drop down and click OK. NOTE: The “vr” prefix on the template name, such as vr_name, may or may not be used. This is a client preference. Emdat Proprietary Page 9 AnyModal Editor for the MT Guide The ShadowScribe Menu This menu will appear each time a ShadowScribe user’s dictation is loaded and contains functions specific to the AnyModal editor. Where applicable you are strongly encouraged to learn and use the shortcut keys as this will help you increase efficiency and productivity. This will automatically place an additional space at the end of a sentence and auto capitalize the first word of the new sentence. After you finish the sentence, add a period hit space then start typing the first letter of the next word. Another space will be added and the first letter of the new word will be capitalized. Note: “Capitalize First Letter of Sentence as You Type” must be enabled in Spelling and Typing Options under the Options menu. 2. Change Playback Highlighter Color. Available only for editing a draft, not available while transcribing. Allows you to change the color of the Playback Highlighter. Emdat Proprietary 11 Page 10 AnyModal Editor for the MT Guide 3. Optimize Dictation Playback. Available only for editing a draft, not available while transcribing. When on, this setting will automatically skip through long pauses of the dictation where there is no dictation such as when the clinician is flipping through a patient’s chart. 4. Attach Selection to Playback Highlighter. Available only for editing a draft, not available while transcribing. Use this option to attach your edit cursor to the playback highlighter. 5. Move Playback to Cursor. Available only for editing a draft, not available while transcribing. Use this option to automatically start the playback from virtually any word you select. 6. Split Transcription at Cursor. Similar to the Emdat Editor Copy Transcription to New Transcription function, this will split the transcription at the cursor location and create a new transcription containing all the content to the right of the cursor. 7. Insert Line Break. By default when the enter key is pressed lines will be double spaced. This option is used to override the double space and create a single line. Note: A known issue exists with this feature. When you hit shift+enter the edit cursor moves one space to the right of the current line instead of below the current line. If you start typing, your text will appear below the current line as it should. 8. Toggle Hidden Text. Available only for editing a draft, not available while transcribing. This option will mark or unmark text as hidden text, placing it in the gray background with the dash line around it. 9. Toggle Section. Inserts or removes a section heading. 10. Toggle Sub-Section. Inserts or removes a subsection heading. 11. Toggle List. Inserts or removes a numbered or bulleted list. 12. Indent List. Indents a list item one level. 13. Unindent List. Unindents a list item one level. 14. Increase Zoom. Increases the size of the text in the editor. The zoom setting will be kept even after logging out of InScribe. This doesn’t affect what is delivered to the client. 15. Decrease Zoom. Decreases the size of the text in the editor. The zoom setting will be kept even after logging out of InScribe. This doesn’t affect what is delivered to the client. 16. Reset Zoom. Restores the text size to the original default setting during install. 17. AutoComplete Word. Automatically inserts suggested word hints you see blow as you type. Hit Tab to insert the word hint. Note: this option requires the ShadowScribe Options Show AutoComplete Word hints enabled. To enable go to the options menu, select ShadowScribe Options and check the “ShowAutoComplete Word hints box”. Failing to listen attentively may lead to making mistakes, such as transcribing a word or phrase incorrectly. When you attentively listen you put extra conscientious effort into listening, tuning your ear to listen to not just the words, but also the context in which the words are spoken. When you put this type of effort into listening, you have a heightened awareness of the dictation which is a powerful tool for the transcriptionist. This tool can help you accurately, efficiently, and quickly transcribe and also help you more easily identify dictator mistakes. A section refers to both the section title and its content. AnyModal Editor displays a section between purple lines. HOW DO I MAKE A NEW SECTION? You can create a new section by typing Ctrl+N. MY SECTION TITLE SHOULD BE BOLD. HOW DO I DO THAT? Formatting of section titles is done automatically when you submit the report. There is no need to bold, or make any other formatting changes, to section titles. I INSERTED A SECTION TITLE AND IT IS DISPLAYED IN RED. WHY? If your section title is red, this means that it is not one of the predefined sections for the document type. Predefined sections are displayed in your drop down when you use Ctrl+N for sections. Usually you only need to add a red section if the dictator spoke a different section that was not in the predefined list. This can happen from time to time. If this happens frequently you should notify your account manager and have them add the section to your pop up list. HOW DO I KNOW WHICH SECTION TITLE TO ADD? When you create a new section title, press Ctrl+N for a list of predefined section titles. Choose the best title from this list. I INSERTED A SECTION TITLE AND IT IS DISPLAYED IN GREEN. WHY? This occurs as you type the letters of the section. As you type, the AnyModal Editor will try to match your characters to one of the predefined sections. If a possible match is close then the section title will appear in green. ONCE I FINISHED TYPING MY SECTION TITLE, IT DISPLAYED IN PURPLE. WHY? A purple section title means that the section is one of the acceptable predefined sections. THERE ARE MANY TITLES TO CHOOSE FROM WHEN I PRESS CTRL+N. DO I NEED TO SCROLL THROUGH ALL OF THEM? No, you may type the first few letters of a section title and then press Ctrl+N. The menu option will begin with those titles that match your selection. THE SECTION TITLE I AM LOOKING FOR IS NOT INCLUDED IN THE OPTIONS WHEN I PRESS CTRL+N. I AM CERTAIN THAT THE TITLE IS PERMISSIBLE IN THE WORK TYPE I AM EDITING. WHAT DO I DO? If the document type was incorrectly identified when the audio was submitted, the wrong list of predefined sections would be assigned to the draft. Change the document type to ensure that the correct predefined section list displays. CREATING A LIST I WANT TO TURN THE CONTENT OF A SECTION INTO A LIST. Emdat Proprietary Page 14 AnyModal Editor for the MT Guide Place your cursor in the section or line you wish to turn into a list and press Ctrl+L. HOW DO I SEPARATE MY PARAGRAPH INTO A LIST? If your paragraph is already a list, simply press Enter after each item. For instance, if your paragraph is a list of medications and dosages, press Enter after each dosage. WHY IS MY LIST BULLETED WHEN MY ACCOUNT WANTS IT ENUMERATED? AnyModal Editor only displays bulleted lists. The appropriate formatting will be applied after you submit the report in the rendering process. HOW DO I CREATE A SUBLEVEL WITHIN MY LIST? You can create a sublevel by typing Ctrl+L on the appropriate line/paragraph. When you type Enter, you will continue your sublevel, much the same as you would in Microsoft Word by hitting the tab key or Ctrl+M. I ACCIDENTALLY CREATED A LIST. HOW DO I TURN IT BACK INTO A PARAGRAPH? It is easy to turn a list back into a paragraph. Simply place your cursor in your listed item and press Ctrl+L. SPEEDING UP AND SLOWING DOWN DICTATION I WANT TO SPEED UP THE DICTATION I AM WORKING ON. HOW DO I DO THAT? To speed up dictation, press Ctrl+T. HOW DO I RETURN PLAYBACK TO NORMAL?
