Principled Practice
Few would disagree that the Practice of Medicine should be principle based.
And yet in the current climate of medicine, many would argue that such
principles are losing emphasis. Fortunately, for some there is no compromise.
At the core of Dr. Dale Erickson’s professional life is just such a commitment
to principle – a lesson which he is enthusiastic about sharing with others. He
did so a couple of weeks ago when he addressed the President’s Council – a
bi-monthly meeting of current and past GAMA President’s.
The talk was titled, The Remarkable Life of Nelson Mandela – Ethical
Lessons Learned. For 30 minutes Dr. Erickson captured our attention as
he traced Mandela’s life from a local native tribal birth to an iconic finale as a
revered world symbol of freedom for the oppressed. Mandela’s principled living arose from the Xhosa tribal philosophy that “One’s humanity derives from
how he interacts with other people”. Well educated in law, Mandela turned his
sights on the cause of the African National Congress for which his Defiance
Campaign protests eventually yielded a charge of treason, a guilty verdict
at the Rivonia Trial and 27 years in prison -19 of which included protracted
periods of isolation on Robben Island.
Years toiling in the purposeless task of “stone smashing” did little to daunt
Mandela’s focus. “I have cherished the ideal of a democratic and free society
in which all persons live together in harmony and with equal opportunities.”
Mandela’s voice was kept alive through that of Archbishop Desmond Tutu
who led innumerable peaceful protests, maintained international attention
and contributed to Mandela’s eventual release.
For some, Mandela’s subsequent election as the President of South Africa in
1994 and recipient of the Nobel Peace Prize thereafter sealed his position as
one of the great men of our time. But it was his principled philosophies that
make him one of the Great Men of All Time. “I have cherished the ideal of
a free society. It is an ideal which I hope to live for, but if need be I am prepared to die for.” To adhere to such a goal required deeper qualities including
Honor, Courage, Wisdom, Perseverance, Integrity, Self-Denial, Reconciliation
and Forgiveness.” These, the core of Principled Living.

Few are better positioned to relate this story than Dr. Erikson. Graduating from Indiana School of Medicine and having a year of residency in Flint
Michigan, Dale soon found himself a mission doctor in Zimbabwe (Rhodesia).

I really enjoyed seeing Dr Shoden’s
perspective on the Electronic Health
Record (EHR) in a private practice office last month. At UNM Health Sciences Center (HSC), we use Cerner
for our EHR. We started fully utilizing
Cerner in about 2000. Cerner is not the
most expensive EHR on the block, but
it does work to a degree.

The system is certainly not intuitive.
There is a lot of repetition. We as physicians are asked to put patient problems/diagnoses under the Problem List tab which has not been well received by many of us. Since
almost all of my patients have complex medical needs with at least 10
or more diagnoses, I must admit that I am remiss in putting problems
in the Problem List tab. It just takes too much time to do this. I do
however, carry over an accurate Problem List/diagnoses in each of
my dictations. Almost everyone now dictates or types their patient
notes except the Pediatric Urgent Care Clinic where up to 200 patients may pass through the clinic in one day.
Inpatient physicians are also ordering labs and diagnostic tests in
Cerner. As an outpatient provider, I still order all of my labs and diagnostic tests on paper (which I prefer). Many of us feel that a lot of
our work in Cerner is clerical and we did not go to medical school to
do clerical work. The newest “thing” now is using “Dragon Speaks”
for dictations and/or dynamic documentation. Dynamic documentation has two columns of text to the page and I find it difficult to find the
information I need in the note. Some of us love Dragon Speaks and
others don’t like it at all.
Honestly, I do like that I can easily click the Appointments tab and see
what appointments are pending, already occurred, and if my patient
showed up for the appointment. E-scripting and messaging are easy.
We do have an “Ad Hoc” tab where we can put in referrals to specialists. We are notified if our patients are in the Emergency Room, admitted to, and discharged from the hospital. One of the best features
is that all referrals, labs, and diagnostics are tracked by our Medical
Assistants electronically. When I see a “RESULT” for my patient, I
am able to document that I contacted the patient in some manner and
then forward this to my Medical Assistant. She then closes the loop in
the medical record for the patient. This has really helped us to make

on the continent.
Schooled with a Masters in Bioethics, Erickson’s experience and exposure to such men as Nelson Mandela
have cemented the principles by which he practices medicine. Mandela’s Nobel Prize speech captures the essence; “Let it never be said by future generations that indifference, cynicism, or selfishness made us fail to live
up to the ideals of humanism,” with recognition that the “beauty of genuine brotherhood and peace (are) more
precious than diamonds or silver or gold.”
With that in mind, Dr. Erickson drives his old Chevy sedan to work every day as he has done many of the past
40 years as a Nephrologist (though he can be seen enjoying a Starbuck’s en route). “My personal mission is
to encourage doctors to establish a genuine professional relationship with their patients, which takes time and
effort. The patient is a real person with hopes, dreams, and fears who wants very much to get well.” Words
that would resonate with Mandela and underpin the Principled Practice of Medicine.

sure we see all results and notify our patients.
Upgrades to the system are always difficult. Of course if you ask for specific things to change or be
added to the system, it is another expense. Often it takes a long time for the changes to be made. Our
last upgrade over the past weekend wiped out my ability to print anything from my computer at work.
We had to patch into the old system before the upgrade to be able to print while they worked out the
kinks from the upgrade. Occasionally the system will go down which wreaks havoc everywhere in the
UNMH system
I believe we all eventually get comfortable with what we are used to. For now Cerner is working for me,
at least until the next upgrade.
UPDATE: I received many kind verbal comments and emails concerning my editorial on “Aging Parents” in June. Early the first week of July, my mother had a bad fall. She recovered some, but by the
end of the week she worsened. She was able to impart some sage advice to my two daughters in a
surprising clear manner over the weekend. She died quietly in her own home looking out the window
into her beloved backyard on July 7, 2015. My oldest daughter read my Editorial at the funeral. I still
see her as the most incredibly strong and bravest woman I have ever met who met death head on with
poise, dignity, and grace. I miss her greatly.

GAMA’s Scholarship
Winner
In today’s world, “Wonder Woman” is not a politically correct term.
But it seems an apt description of GAMA’s first La Tierra Segrada
Society Scholarship Recipient, Julie Collette Culkins.
Julie is a second year medical student at UNM School of Medicine.
Her main specialty interest started as psychiatry, but her rotation
experiences have opened up her interest to possibly other specialties.
Julie has a B.A. in Biology from UNM where she achieved Summa
Cum Laude Honors. She has extensive teaching experience, including programs with the University of St. Francis Physician Assistant program, the UNMSOM Emergency Department, and the
UNM School of Biology.

Leadership skills are very apparent. When Julie sees something that
needs to be done, she does it. She was founder and President of the
UNM MD/PhD Organization, served as President of Alpha Epsilon Delta Pre-health Honor Society and in leadership roles with the Biology Undergraduate Society.
As a community volunteer, Julie has helped organize the Native Health Initiative, PIE Community Project, “What
is your medicine?” She has served on several UNM initiatives including the UNMSOM yearbook committee,
UNMSOM Continuing Quality of Care project and is a Big Sister with Big Brothers/Big Sisters of New Mexico.
Julie is the mother of two and wife of a disabled veteran. She is truly committed to the medical profession. Julie
stated, as our knowledge of the human body and mind expands, I want to make positive changes in the way
we practice medicine through research. New Mexico has always been my home. I am here to stay. Integrating
family life and medicine is very important to me. I want to be both an exceptional mother and physician.

Sophia Waara Promoted
to GAMA Deputy
Director
Congratulations to Sophia Waara on her recent promotion to
the position of GAMA Deputy Director. In her new role, she
will work closely with the Board of Trustees and the Executive
Director to promote the GAMA mission, provide increased services to our members, and to strengthen administrative support
of the organization.
Sophia Waara joined the GAMA organization in September
2013 as the Manager of Medical and Commercial Communications (MACC), the for-profit communications company owned
by GAMA. MACC is a full-service company that provides
answering service assistance to over 900 clients in the health
and commercial fields.
Sophia Waara
Sophia has coordinated the expansion of MACCs client base, overseen major upgrades in MACC technical
support, and manages seventeen MACC operators and supervisors. She coordinated the HIPAA Compliance audit and has implanted stronger security measures to protect client and patient information. She will
continue to manage MACC.
Sophia came to GAMA/MACC from the financial world with a strong background in program and project
management, marketing and advertising, client management, administration and retention, and data analysis. One of her largest projects at GAMA was the coordination of a Gross Receipts Tax data collection project that provided data to GAMA leadership and to the NM Taxation & Revenue Department.
From the University of West Florida, Sophia has an M.A., Leadership Communication with Health Leadership and Health Ethics Certificates. She has a B.A. in Communications with a specialty in advertising.
Sophia lives in Rio Rancho with her husband, Christian, a member of the United States Marine Corps and
their ten month old daughter, Taylor. They also have two dogs as members of their family.
Sophia brings an incredible work ethic and professional background to GAMA/MACC. Her leadership style
