SUMMARY: Benefits & Entitlement Specialist assists clients with the acquisition of
Social Security benefits (SSI/SSDI) and Medicaid/OHP benefits. This includes meeting
with clients, obtaining and reviewing medical records and lay evidence, and filing Social
Security claims using the Social Security Administration’s on-line application system.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1.
Screen clients for potential eligibility for disability SSI/SSDI/Medicaid/OHP
benefits.
2.
File clients Social Security Disability (SSD) and Supplemental Security Income
(SSI) applications and appeals accurately and timely.
3.
Establish a list of case needs for disability claimants.
4.
Obtain all pertinent medical records and lay evidence for review and submission
to the legal record.
5.
Research applicable rules and regulations
6.
Interview claimants for medical and functional summary reports.
7.
Provide outreach and transportation assistance to disabled clients experiencing
homelessness and mental illness.
8.
Attend disability related appointments with doctors and the Social Security
Administration offices.
9.
Communicate with medical providers and case managers.
10.
Maintain statistical spreadsheets and progress notes.
11.
Remain up-to-date with changes in OHP/Medicaid and Social Security Disability
benefits.
12.
Attend all scheduled staff meetings and staff training as applicable.
13.
Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR
Part 2, and to CCC policies and agreements regarding confidentiality, privacy,
and security. Support compliance with all privacy and security requirements
pursuant to community partners’ and outside providers’ patient confidentiality
agreements, including privacy and security requirements for EMR access. This
includes immediate reporting of any PHI breach of CCC or outside provider
health records to the CCC Legal and Quality departments, as well as to the
program administrator.

MINIMUM QUALIFICATIONS:
1.
High school diploma or GED equivalent required.
2.
Associate degree or higher or other post-high school professional training
preferred.
3.
Bilingual in English and Spanish preferred.
4.
Must be able to understand pertinent information from medical records.
5.
Must have experience in case management or advocating for clients.
6.
Computer literacy.
7.
Recovering chemically dependent person must be currently clean and sober.
8.
Will be required to carry a cell phone for work purposes. Cell phone to be
provided by Central City Concern.
9.
Must pass pre-employment drug screen, TB test, and background check.
10.
Must be able to occasionally lift items of approximately 25 pounds.
11.
Must have and maintain a valid driver’s license, and be able to pass a driving
record check as determined by Central City Concern criteria and also must have
access to a vehicle and maintain personal automobile insurance which meets
Central City Concern criteria.
12.
Must adhere to agency’s non-discrimination policies.
