HOW TO FILL OUT A 4-H ACTIVITY REPORT
Listed is an explanation of each page of this 4-H Activity Report. Please read carefully before filling out the pages of this
report. Include all the activities you did each year to keep your report complete and accurate. You will use this activity
report when you apply for any award, scholarship, medal or certificate in Ulster County.
SECTION 1 – SUBJECT AREAS FOR 4-H PROJECTS:
List one of the subject areas below or a subject area from the Ulster County Fair Book on the top of your 4-H Project
page. Have at least one page for each subject area. Put the year, name of the project and the awards (blue, red or white,
etc.) received if entered in the County and/or State Fair. Leave blank if not entered in these events.
