CONSOLIDATED TEXT Approved by Rector’s directive No 328 of 20 December 2013 Procedure for Preparation and Issue of Continuing Education Documents 1. General provisions 1.1. This procedure establishes the procedure and forms for continuing education diplomas, diploma supplements, certificates and diploma duplicates at Tallinn University of Technology. 1.2. A diploma shall be issued to a person who has completed a continuing education course to its full extent. A diploma shall be issued together with a diploma supplement, which shall include the academic performance results. 1.3. A diploma duplicate and its supplement shall be issued to a person if the diploma and/or its supplement has been lost, destroyed or damaged. 1.4. A certificate shall be issued to a person who has participated in a continuing education course or has registered for a continuing education course. 1.5. Diplomas, diploma supplements and certificates shall be drawn up in Estonian. By a decision of the head of the structural unit organising the continuing education course a diploma supplement and/or certificate may be drawn up also in English in addition to the documents in Estonian. The continuing education course participants who have completed a curriculum in English shall be issued documents in English. 2. Issue of diplomas and diploma supplements 2.1. A diploma shall be drawn up on headed form supplied with TUT embossed stamp intended for such purpose. (Annex 1). 2.2. Open University shall issue headed diploma forms to the structural unit organising the continuing education course on the basis of a written application. The head of the structural unit organising the continuing education course shall be responsible for preservation and proper use of the headed forms issued. 2.3. The following data shall be entered on the headed diploma form in accordance with the form (Annex 2): 2.3.1. first name and surname of the continuing education course participant; 2.3.2. personal identification code, in the absence thereof date of birth of the continuing education course participant; 2.3.3. code of the continuing education curriculum or formal education course; 2.3.4. title of the subject of the continuing education curriculum or formal education course; 2.3.5. period of study; 2.3.6. learning outcomes; 2.3.7. brief description of the content (if necessary); 2.3.8. total volume in hours and credit points; 2.3.9. name of the structural unit responsible for the content; 2.3.10. name of the structural unit, which organised the continuing education course; 2.3.11. reference to the funder and/or cooperation partner (if necessary); 2.3.12. reference to an Annex (if available); 2.3.13. signatories: first name, surname, structural unit, official title; 2.3.14. place and date of issue of the diploma. 2 2.4. 2.5. 2.6. 2.6.1. 2.6.2. 2.7. 2.8. 2.8.1. 2.8.2. 3. 3.1. 3.2. 3.3. 3.4. 4. 4.1. 4.1.1. 4.1.2. 4.1.3. 4.2. 4.2.1. A diploma supplement in Estonian shall be drawn up on the university’s headed form. A diploma supplement in English shall be drawn up on the university's bilingual headed form. Data shall be entered on a diploma supplement in accordance with the form (Annex 3) A diploma and a diploma supplement shall be signed as follows: in case of a continuing education curriculum by the head of the structural unit responsible for the content and by the organiser of the continuing education course; in case of open studies by the Head of Open University or director of the educational institution and the coordinator of the open studies. A diploma and a diploma supplement shall be certified by the TUT stamp. A diploma and a diploma supplement issued by an educational institution shall be certified by the stamp of the relevant educational institution. A diploma and a diploma supplement shall be issued: to the person who completed a continuing education curriculum by the structural unit who organised the continuing education course; to the person who completed open studies by Open University or the educational institution. Issue of a diploma duplicates and their supplements For issue of a diploma duplicate and its supplement a person shall submit a written application to Open University. If a diploma or its supplement has been damaged, it shall be attached to the application. A diploma duplicate shall be drawn up on the diploma form. The text of the original document and the date of issue shall be entered on the form. In the centre at the bottom of the page the following shall be printed: "duplicate issued" and the date of issue of the duplicate. "DUPLICATE of diploma No" and the number of the original document shall be printed in the upper right corner of the form (Annex 4). A duplicate supplement shall be drawn up in compliance with the provisions of clauses 2.4-2.5 of this document. A duplicate and its supplement shall be signed by the Head of Open University and the coordinator of the continuing education course or open studies. A duplicate and its supplement shall be issued by Open University. Issue of a certificates A certificate shall be issued concerning participation in a continuing education course based on a continuing education curriculum, if knowledge and skills were not assessed in the course of the continuing education course or if the continuing education course participant failed to meet the requirements for completion of the curriculum. A certificate shall be drawn up as follows: the data shall be entered on the certificate in accordance with the form (Annex 5); the certificate shall be signed by the head of the structural unit who organised the continuing education course; the certificate shall be issued by the structural unit who coordinated the continuing education course. A status certificate shall be issued concerning registration for a continuing education course conducted based on a continuing education curriculum. A status certificate shall be drawn up as follows: the data shall be entered on the certificate in accordance with the form (Annex 6); 3 4.2.2. the certificate shall be signed by the head of the structural unit who organised the continuing education course; 4.2.3. the certificate shall be issued by the structural unit who coordinated the continuing education course. 4.3. An open studies course participant shall be issued a transcript of records, whereto the academic performance results shall be entered. A transcript of records shall be drawn up as follows: 4.3.1. the data shall be entered on the certificate in accordance with the form (Annex 7); 4.3.2. the certificate shall be signed by the coordinator of open studies; 4.3.3. the certificate shall be issued by Open University or the educational institution. 4.4. An open studies course participant shall be issued a status certificate concerning registration for open studies. A status certificate shall be drawn up as follows: 4.4.1. the data shall be entered on the certificate in accordance with the form (Annex 8); 4.4.2. the certificate shall be signed by the coordinator of open studies; 4.4.3. the certificate shall be issued by Open University or College. 4.5. The certificates specified in clauses 4.1-4.4 shall be drawn up on the university’s headed form. 4.6. If a certificate specified in clauses 4.1-4.4 is lost, destroyed or damaged, a new certificate shall be issued to the person. 5. 5.1. 5.2. 5.3. 5.4. 5.5. 5.6. Registration and issue of continuing education documents and acts of their issue The diplomas shall be registered in the study information system register of diplomas based on a uniform numbering system managed by Open University. The issuer of a diploma shall submit the act of issue of diplomas to Open University within one week as of issue of a certificate. An act shall be drawn up in accordance with the form (Annex 9). A copy of the diploma and its supplement (if available) issued shall be attached to the act. Damaged headed forms of diplomas shall be returned to Open University for safe disposal together with the act and the numbers of the headed forms shall be declared invalid in the register of diplomas of the study information system. An act shall be prepared concerning issue of a diploma duplicate and issue of a duplicate shall be registered in the register of diplomas with a notation “Duplicate”. Certificates shall be registered in the TUT study information system based on a uniform numbering system.
